控氧灌注抑制犬肺局部及全身炎症因子表达

摘要 目的:探讨控氧灌注对体外循环犬肺局部及全身炎症反应及肺损伤的影响。方法:14只健康犬随机分为对照(IR)组和控氧灌注(OCR)组,每组7只,开胸建立体外循环,经过60min升主动脉阻断和90min再灌注。OCR组在主动脉开放瞬间FiO2为40%,随后每5min依次上调10%,最后达80%并保持至实验结束,IR组开放后再灌注全程FiO2保持80%不变。在开胸后(T1)、主动脉开放后25min(T2)、90min(T3)分别留取血液标本及肺标本,检测血白介素6(IL-6)和肿瘤坏死因子α(TNF-α);肺组织NF-κB蛋白表达、髓过氧化物酶(MPO)活性、丙二醛(MDA)含量;肺组织干湿重比及光镜下病理变化。结果:在T2和T3时点,OCR组肺组织NF-κB蛋白表达显著低于IR组(T2,0.321±0.116vs.0.547±0.067;T3,0.318±0.071vs.0.525±0.025,P<0.05),MPO及MDA也较低。血IL-6(T2,166.7±28vs.1088.3±300;T3,168.1±24vs.1052.5±315,P<0.05)和TNF-α(T2,74.9±6vs.503.8±144;T3,73.2±6vs.451.2±130,P<0.05)也显著低于IR组。OCR组肺组织干湿重比在T2和T3时点高于IR组,病理改变也较轻。结论:控氧灌注降低犬肺局部及全身炎症因子表达,减轻肺损伤。
