超声弹性成像应变率比值法与弹性成像分级法在甲状腺良、恶性占位性病变诊断中的应用 被引量:10

摘要 [目的]比较超声弹性成像应变率比值法及弹性成像分级法在甲状腺良、恶性占位性病变中的诊断价值.[方法]对经手术病理检查证实的126例163个甲状腺结节的超声弹性图像进行弹性分级法及弹性应变率比值法分析,利用ROC曲线评价其诊断价值.比较2种方法的特异性、敏感性及准确性.[结果]超声弹性应变率比值法及弹性分级法判断甲状腺占位性病灶的ROC曲线下面积分别为0.927和0.895.2者判断甲状腺良、恶性病灶的敏感性、特异性、准确率分别为87.5%,92.0%,90.2%和81.3%,89.9%,83.4%.二者比较弹性比值法的特异性及准确性均高于弹性分级法,但差异均无统计学意义(P>0.05).[结论]超声弹性应变率比值法及弹性分级法都是甲状腺良、恶性占位性病变判断的新的可行性诊断方法,其中超声弹性成像应变率比值法略优于弹性分级法. OBJECTIVE To compare the diagnostic value of ultrasonic elastography strain rate ratio method and elasticity imaging classification method for diagnosis of thyroid benign and malignancy ndules. METHODS The ultrasound elastography images of 163 of thyroid nodules in 126 cases diagnosed by pathology were analyzed by elasticity score and the elastic strain rate ratio, and its diagnostic value was evaluated by using ROC curves, and the specificity, sensitivity and accuracy were compared between two methods. RESULTS The occupying areas under ROC curve (Az) of ultrasonic elastic strain rate ratio method and elastic score to determine thyroid lesions were 0. 927 and 0. 895, and the sensitivity, specificity, and accuracy of both two methods for differential diagnosis for thyroid benign and malignant lesions were 87.5%, 92.0%, 90, 2% and 81.3%, 89.9%, 83.4%, respectively. In two methods, specificity, accuracy of the elasticity ratio method was higher than that of elastic grading, but the differences had no statistically significant (P 〉 0.05). CONCLUSION Both ultrasonic elastic strain rate ratio method and elastic classification are new feasibility methods for differential diagnosis of thyroid benign and malignancy, and the ultrasonic elastography strain rate ratio method is a little better than the elastic classification method.
