高原地区不同剂量阿司匹林对脑梗死患者D-二聚体、hs-CRP的影响研究

The Effect of Different Dosage of Aspirin on d-dimer,hs-CRP and Prognosis in Patients with Cerebral Infarction

目的:探讨高原地区脑梗死(CI)急性期使用不同剂量阿司匹林(ASP)对患者高敏C反应蛋白(hs-CRP)、D-二聚体的影响。方法:将90例来自高原地区的CI患者随机分为ASP 50mg组(A组)、100mg组(B组)、300mg组(C组),检测3组治疗前后血清hs-CRP、D-二聚体及NINHSS评分变化,并与正常对照组(D组)比较。结果:治疗前A、B、C组hs-CRP、D-二聚体明显高于对照组(P<0.001),治疗2周后,hs-CRP降低幅度C组>B组>A组(P<0.05或0.01),D-二聚体降低幅度C组>B组>A组(P<0.05或0.01),显效率C组>B组>A组(P<0.05或0.01)。结论:脑梗死急性期hs-CRP、D-二聚体明显升高,发病2周内给予大剂量阿司匹林可以使D-二聚体及hs-CRP下降更明显,疗效优于小剂量组。

Objective：To compare the effect of different dosage of aspirin on d-dimer,C-reactive protein（hs-CRP） and prognosis in patients with acute cerebral infarction at high altitude.Method：90 patients from high altitude（mean 3 300m） with acute cerebral infarction were randomLy assigned to group A,B and C（each 30 cases） to receive aspirin 50mg/d,100mg/d and 300mg/d,half-hour oral after dinner for two weeks,respectively;group D served as the healthy control group（30 cases）.There was no significance difference in NIHSS scores among the four groups before treatment（P0.05）.All subjects＇ serum level of hs-CRP and D-dimer were detected respectively by scattering turbidimetry and immunonephelometry;and NIHSS score was recorded before and after treatment.Results：Before treatment,the serum level of hs-CRP and D-dimer in group A,B and C was significantly higher than those in control group（P0.01）.After two weeks of treatment,the decreased degree of hs-CRP and D-dimer was group C group B group A（P0.05 or 0.01）,as well the obvious effective rate.Conclusions：The serum level of hs-CRP and D-dimer was increased significantly at occurrence of acute cerebral infarction;but they can be decrease significantly by treatment with large dosage of aspirin onset within two weeks,and the prognosis is better than treatment with low dosage of asprin at high altitude.
