猪尾导管在经导管动脉栓塞术治疗大咯血中的应用

摘要 目的:分析大咯血的复杂供血血管,评价猪尾巴导管在介入治疗中的应用价值。材料与方法:回顾分析近十年来收治108例大咯血病例,其中男性86例,女性22例,平均年龄(42±10)。所有患者均常规采用猪尾导管先端分别置于胸主动脉及腹主动脉造影,然后再栓塞责任或可疑血管。结果:术后第一天停止咯新鲜血痰86例(80%,86/108),术后第二天停止咯新鲜血痰16例(15%,16/108),好转4例(3.70%,4/108),2例无效(1.85%,2/108),总有效率98%以上。术后出现症状性异位栓塞5例,未发生死亡及脊髓损伤等严重并发症病例。结论:大咯血责任血管较为复杂,猪尾巴导管的应用可见大大缩短手术时间,防止漏栓情况的发生。 Purpose: To analyze the complexity of the blood vessels that supply of massive hemoptysis, the applica- tion value of the pigtail catheter in the interventional treatment. Materials and Methods: Retrospective analysis over the past decade treated 108 cases of massive hemoptysis cases, 86 cases were male and 22 females, average age (42 ± 10). All patients underwent conventional pigtail catheter tip is placed in the thoracic aorta and abdominal aorta, and then embolism re- sponsibility or suspicious vessels. Results: Stop after the first day of slightly fresh and bloody sputum in 86 cases (80% , 86/108 ) , second day after surgery to stop slightly fresh bloody sputum in 16 cases ( 15% , 16/108), improved in 4 cases (3. 70% , 4/108invalid) , 2 cases (1. 85% , 2/108) , there is always more than 98% of efficiency. Postoperative symptomatic ectopic embolism five cases, not death and spinal cord injury in cases of serious complications. Conclusion : of massive he- moptysis the blood vessels that supply the application of the pigtail catheter is seen to greatly shorten the operative time, to pre- vent the drain bolt from happening
