儿童原发性免疫缺陷合并马内菲青霉菌病1例并文献复习

Primary immune deficiency associated with Penicilliosis marneffei in children: a case report and review of the literature

目的提高儿童马内菲青霉菌病的临床诊断水平。方法回顾分析1例原发性免疫缺陷病合并马内菲青霉菌病患儿的临床资料并进行相关文献复习。结果反复发热、"肺炎"、肝脾进行性肿大、皮疹、免疫功能低下等为马内菲青霉菌病的基本特征,骨髓细胞形态学检查见马内菲青霉菌及血液真菌培养发现马内菲青霉菌生长是确诊依据。结论认识马内菲青霉菌病的临床特征,避免漏诊、误诊。

Objective To improve the clinical diagnostic level of Penicilliosis marneffei in children. Methods Clinical data of an infant with primary immune deficiency combined with Penicilliosis marneffei were observed and analyzed. Relevant literature were reviewed. Results The main characteristics of Penicilliosis marneffei are high fever, ＇pneumonia＇, progressive hepatomegaly and splenomegaly, skin lesion and immune function deficiency. The definite diagnosis for this disease can be established by finding penicillium marneffei in bone marrow smears and blood cultivation. Conclusions The clinical characteristics of Penicilliosis marneffei should be known to avoid missed diagnosis or misdiagnosis.
