陶瓷对陶瓷人工髋关节置换术治疗髋关节疾患 被引量:7

摘要 目的:探讨陶瓷头对陶瓷内衬人工髋关节置换术治疗髋关节疾患的临床疗效与安全性。方法:2001年10月至2007年10月,采用陶瓷头对陶瓷内衬人工髋关节置换术治疗髋关节疾患患者30例36髋,男11例,女19例。年龄21~65岁,中位数53岁。发育性髋关节脱位5例,股骨头坏死5例,骨性关节炎4例,强直性脊柱炎6例,类风湿关节炎3例,GardenⅣ型股骨颈骨折7例。合并高血压5例,糖尿病7例。术前Harris评分为(41.7±3.7)分。术后随访观察并发症发生情况及患肢功能恢复情况。结果:27例32髋获得随访,随访时间44~84个月,中位数58个月;早期死亡l例(双髋),失访2例2髋。术后出现外伤性股骨假体周围骨折而行人工关节翻修术3例;术后发生陶瓷内衬破裂而行人工关节翻修术2例;术后出现"咯吱"异响但未行翻修术3例;其余患者术后无感染、神经血管损伤、关节脱位等并发症发生。末次随访时所有患者X线片示髋关节假体在位,金属臼杯、股骨假体与宿主骨嵌合良好,无明显松动。髋关节活动度均无明显受限,患侧髋关节均无疼痛,均无需使用行走辅助工具。Harris评分提高至(89.1±2.9)分。按照改良髋关节Harris评分标准评定疗效,优10例12髋,良14例17髋,中2例2髋,差1例1髋。结论:采用陶瓷头对陶瓷内衬人工髋关节置换术治疗髋关节疾患,可以明显提高患者的生活质量,并发症少,临床疗效令人满意,值得临床推广应用,但需要避免陶瓷界面特有的并发症,假体使用需要良好的临床技术。
