低温等离子消融术治疗变应性鼻炎伴鼻息肉疗效分析

Analysis of Therapeutic Effect of Low-temperature Plasma Ablation on Treatment of Allergic Rhinitis with Nasal Polyp

目的探讨低温等离子消融术治疗变应性鼻炎伴鼻息肉的疗效及不良反应。方法选取变应性鼻炎伴鼻息肉患者116例,58例为变应性鼻炎伴鼻息肉Ⅰ期,另58例为变应性鼻炎伴鼻息肉Ⅱ期或Ⅲ期。2组均采用鼻内镜下低温等离子消融术。结果 I期组总有效率96.6%,治愈率为13.8%;Ⅱ期、Ⅲ期组总有效率为77.6%,治愈率为1.7%。2组总有效率比较差异有统计学意义(χ2=9.26,p<0.01),治愈率比较差异也有统计学意义(χ2=4.34,p<0.05)。结论鼻内镜下低温等离子消融术能改善、恢复鼻通气、窦通气和保证鼻窦引流通道,对解剖结构异常者则难以纠正,对变应性鼻炎伴严重鼻息肉者效果差。

Objective To discuss the therapeutic effect and adverse reaction of low-temperature plasma ablation on treatment of allergic rhinitis with nasal polyp. Method 116 cases of patients suffering from allergic rhinitis with nasal polyp were chosen, of which 58 cases were in phase I of allergic rhinitis with nasal polyp while an- other 58 cases were in phase Ⅱ or Ⅲ of allergic rhinitis with nasal polyp, and the two groups were treated with low-temperature plasma ablation under nasal endoscope. Results Total effective rate of the group in phase I was 96.6% and the curative rate was 13.8% ; total effective rate of the group in phase Ⅱ or phase Ⅲ was 77.6% and the curative rate was 1.7%. Differences in total effective rate between the two groups were statistically significant （Z2= 9.26, p〈0.01）, and differences in the curative rate were statistically significant （X2 =4.34, p〈0.05） as well. ConcLusion Low-temperature plasma ablation under nasal endoscope can im- prove and recover nasal airflow, sinus ventilation and guarantee the sinus drainage, but it has difficulty correc- ting patients with abnormal anatomical structures and has poor effects on patients suffering from allergic rhini- tis with severe nasal polyp.
