摘要 目的研究改良脱机指数(MEI)、浅快呼吸指数(RSBI)及顺应性-率-氧合-压力指数(CROPI)在预测新生儿呼吸机脱机中的价值。方法采用回顾性分析方法,分析152例呼吸机治疗新生儿脱机结果与上述三项指标之间的关系,并比较上述三项指标预测不同脱机结果的能力。结果三项指标在预测呼吸机治疗新生儿脱机成功的特异度均能达到90%以上;其中CROPI灵敏度及特异度均最高(灵敏度83.6%,特异度93.4%),其截断值为0.194,受试者工作曲线下面积为0.92。结论 CROPI在预测呼吸机治疗新生儿脱机中可能具有较好的应用价值。 Objectives To evaluate the predictive values of modified extubation index (MEI), rapid shallow breath- ing index (RSBI) and compliance rate oxygenation and pressure index (CROPI) in newborns undergoing ventilation therapy. Methods The relationships of outcome and 3 indexes (MEI, RSBI and CROPI) of 152 neonatal treated with ventilation therapy were retrospectively analyzed. The values of these 3 indexes were compared and evaluated on successful extubation. Results All the 3 indexes showed above 90% of specificity on predicting successful extubation, and CROPI showed the highest sen- sitivity (83.6%) and specificity (93.44%) with the area under receiver operating characteristic curve was 0.92. The cut-off of CROPI was 0.194. Conclusions CROPI could be the better index for predicting successful extubation in children with ventila-tion therapy.
