摘要
目的:研究溃疡性结肠炎(UC)患者肠黏膜蛋白酶激活受体2(PAR-2)及环氧合酶2(COX-2)表达情况及在UC发病机制中的作用。方法:UC患者36例,对照组33例,均行内镜检查并活检肠黏膜组织,应用组织病理学及免疫组织化学分析组织中PAR-2及COX-2表达情况。结果:UC患者肠黏膜PAR-2和COX-2表达均高于对照组(P<0.05)。重度UC组PAR-2及COX-2表达高于中度、轻度及对照组(P<0.05);病理Ⅰ~Ⅳ级病变肠黏膜中PAR-2表达均高于对照组(P<0.05),Ⅲ~Ⅳ级高于Ⅰ~Ⅱ级(P<0.05)。病理Ⅲ~Ⅳ级COX-2表达明显高于Ⅰ~Ⅱ级及对照组(P<0.05,P<0.01)。UC患者正规治疗,进入缓解期后两者表达明显下降(P<0.01)。在肠黏膜组织中PAR-2表达与COX-2表达呈正相关(r=0.501,P<0.01);均与UC病理分级呈正相关(P<0.05)。结论:UC的发生发展与PAR-2及COX-2过度表达密切相关,两者共同促进炎症的发生发展。
Objective: To investigate the expression of PAR-2 and COX-2 in intestinal mu- cosa of patients with ulcerative colitis, and the possible roles of PAR-2 and COX-2 in the patho- genesis of UC. Methods: Experimental colonic tissues were obtained from 36 patients with UC un- dergoing colonoscopy. Normal colon tissues were obtained from 33 healthy controls (UC patients with lesion mucosa, control group with normal mucosa). Histopathological examination and Im- munohistochemistry were used to determine the expression of PAR-2 and COX-2. Result: Se- vere UC group PAR-2 and COX-2 expression was higher than moderate, mild and control group. Expression of PAR-2 in the mucosa of patients with inflammation grade I -IV was significantly higher than that of the healthy control(P〈0.05). Grade III ~ IV exhibited significant higher expression than that of grade I - II (P〈0.05). The patients with inflammation grade tll- IV exhibited significant higher expression of COX-2 than that of grade I -II and the healthy control (P〈0.05 and P〈O.01), however the patients with inflammation grade t - II had no significant different expression of COX-2 (P〉0.05).The expression of PAR-2 and COX-2 in the colonic lesions mucosa of patients with UC was significant higher than that of the no lesions mucosa (P〈0.01).The expression of PAR-2 and COX-2 was significant lower after regular systemic treatment (P〈0.01). The expression of PAR-2 was positively correlated with the expression of COX-2(r=0.501, P〈0.01 ),and also positively corre- Lated with the pathological grade of UC (P〈O.05). Conclusion: The occurrence of UC correlated with the increased expression of PAR-2 and COX-2. PAR-2 and COX-2 had a close relationship and could jointly promote the development of inflammation.
