摘要
目的随着心脏外科学的发展,先天性心脏病患者越来越多地选择手术治疗。文中探讨64层螺旋CT在先天性心脏病术后评估中的应用。方法对16例先天性心脏病患者术后CT资料进行分析,并与超声心动图对照。所有病例均行多平面重建和容积再现等三维重建后处理,并行超声心动图检查。结果 16例术后患者中,法洛氏四联征矫治术5例、动脉导管未闭结扎术2例、主动脉-肺动脉人工血管术2例、室间隔缺损修补术1例、肺动脉环扎术1例、右肺动脉-肺动脉人工血管及动脉导管未闭结扎术1例、Gleen术1例、全腔静脉吻合术1例、主动脉弓离断矫治术1例、心肺移植1例。64层螺旋CT准确显示了全部病例手术部位的解剖结构。结论 64层螺旋CT结合三维重建是一种先天性心脏病术后评估的重要方法。
Objective With the development of cardiosurgery, surgical strategy is more and more preferred in the manage- ment of congenital heart disease (CHD). This study aimed to investigate the clinical application of 64-slice CT in the postoperative evaluation of congenital cardiovascular abnormalities. Methods We retrospectively analyzed the CT images of 16 postoperative pa- tients with congenital cardiovascular abnormalities. All the patients received multi-planar reconstruction, curved-planar reconstruction, volume render, maximum intensity projection, minimum intensity projection, and ultrasound echocardiography. Results Of the 16 patients, 5 underwent total correction of Tetralogy of Fallot, 2 ligation of the ductus arteriosus, 2 aorta-pulmonary artificial vascular connection, 1 repair of ventricular septal defect, 1 banding of pulmonary artery, 1 right pulmonary artery-pulmonary artery artificial vascular connection and ligation of patent ductus arteriosus, 1 Gleen procedure, 1 anastomosis of both vena cava, 1 repair of interrupt- ed aortic arch, and 1 combined heart and lung transplantation. The operative sites and their anatomic structures of the 16 patients were all precisely manifested by 64-slice CT. Conclusion Combined use of 64-slice CT and 3-D reconstruction is an important method for postoperative evaluation of congenital cardiovascular abnormalities.
