健康体检人群中Hp感染与糖代谢异常及胰岛素抵抗的相关性分析

Correlation analysis of Hp infection with abnormal glucose metabolism and insulin resistance in healthy population

目的探讨健康人群中Hp感染与糖代谢紊乱及胰岛素抵抗的关系。方法检测118例健康体检者的Hp感染情况,以及空腹及餐后2h血糖、胰岛素、糖化血红蛋白、血脂、体重指数等指标,计算反映胰岛素抵抗和胰岛素分泌功能的HOMA-IR、HOMA-IS。对Hp阳性组和Hp阴性组的糖代谢及胰岛素抵抗情况进行分析。结果共检出Hp感染者57例,未感染者61例,Hp感染率达48.31%。Hp感染与性别、年龄无明显相关。57例Hp阳性者中血糖异常者35例(61.4%),而61例Hp阴性者中血糖异常者22例(36.23%)。Hp阳性组的FPG、2hPG和HbA1c较Hp阴性组升高,差异存在统计学意义(P<0.01或P<0.05)。反映胰岛素抵抗的HOMA-IR指数,在两组间差异也有统计学意义P=0.015)。而空腹胰岛素(FIN)和HOMA-IS虽然在Hp阳性组较Hp阴性组有升高,但差异无统计学意义(P>0.05)。HDL-C在Hp阳性组较Hp阴性组有明显降低(P<0.05)。结论 Hp感染与血糖代谢异常有关,且能增加胰岛素抵抗。

Objective To investigate the relationship between infection of Hp and disorders of glucose metabolism and insulin resistance in healthy population. Methods To detect 118 cases undertaken health examination of the situation of Hp infection, as well as fasting and 2 hours postprandial glucose, insulin, HbAle, lipids profiles, body mass index, calculate the insulin resistance and insulin secretion function of the HOMA-IR, HOMA-IS. For Hp positive and Hp negative group of glucose metabolism and insulin resistance were compared. Results In tatol 118 cases, Hp infection in 57 cases, no infection in 61 cases, Hp infection rate of 48.31%. Hp infection is not associated with sex, age. 35 cases （61.4%） of IGR were found in Hp positive group, while 22 cases（ 36. 23%） of IGR in Hp negative group. FPG, 2hPG and HbA1c level in Hp positive group was elevated compared with Hp negative group, which was significant difference （ P〈0.01 or P〈0.05 ）. HOMA-IR, which reflecting insulin resistance index, between the two groups had significant difference （ P = 0. 015 ）. While FIN and HOMA-IS increased in Hp positive group, but no significant difference （ P〉0. 05 ）. HDL-C in Hp positive group had significantly decreased compared with Hp negative group （ P〈 0. 05 ）. Conclusion Hp infection is related to glucose metabolic abnormalities, and promote insulin resistance.
