单向式全电视胸腔镜下肺叶切除术与常规开胸肺叶切除术治疗非小细胞肺癌的对比分析 被引量:10

摘要 目的对比单向式全电视胸腔镜下肺叶切除术与常规开胸肺叶切除术治疗非小细胞肺癌的疗效。方法选择性回顾66例确诊非小细胞肺癌,行肺叶切除+淋巴结清扫治疗的患者,将其中28例施行单向式全电视胸腔镜下肺叶切除手术分为A组,38例施行开胸肺叶切除术分为B组,两组对比分析。结果 A组手术时间,术后胸液量,拔管时间,术后并发症,平均住院时间,术中失血量,术后疼痛程度(vision analogue score,VAS),切口长度均低于B组(P<0.05);淋巴结清扫数目2组差异无统计学意义(P>0.05)。结论单向式全电视胸腔镜下肺叶切除术治疗非小细胞肺癌创伤更小,手术操作简单,疗效确切。 Objective To contrastive efficace of single direction c-VATS lobectomy and common thoraeotomy lobectomy in treatment of non-small cell lung cancer. Methods Retrospetive analysis of 66 cases of lobectomy and lymph node dissection in the treatment of non-small cell lung cancer. Group A were treated with single direction c-VATS lobectomy, Group B were trealed with common thoraeotomy lobectomy, contrastive analysis of two groups. Results Operation time , chest fluid after surgery, chesttube duration, extubate time, rate of earlier period pulmonary complications and average stay of group A were lower than those of group A ( all P 〈 0.05 ), blood loss in surgery, V AS ,the length of cut, were lower than those of group A ( all P 〈 0. 0 l ). tow groups in No. of lymph node dissection showed no significant difference(P 〉 0. 05). Conclusion Because of the lower wound, more simple and Curative effect, single direction c-VATS lobectomy is superior than common thoraeotomy lobectomy in the treatment of non-small cell lung cancer.
