糖尿病对慢性阻塞性肺疾病患者肺功能血气分析的影响

Effect of lung function and blood gas analysis in patients with chronic obstructive pulmonary disease combined with diabetes mellitus

目的探讨慢性阻塞性肺疾病(COPD)与COPD合并糖尿病肺功能及血气的差异。方法 A组为COPD患者22例,B组为COPD合并组糖尿病病程10年以下患者21例,C组为COPD合并组糖尿病10年以上患者18例。所有患者均行肺功能(VC%、FEV1%、FVC%、FEF 25%～75%、DLCO、DLCO/VA、)及血气分析(PaO2、PaCO2)检测,结果行方差分析。结果 C组VC%、DLCO、DLCO/VA、PaO2显著低于A组及B组,差异均有统计学意义(P<0.05)。结论糖尿病病程10年以上COPD患者的肺活量、弥散功能明显减退,肺脏可能为糖尿病靶器官。

Objective To explore the differences of lung function and blood gas analysis in patients with chronic obstruc- tive pulmonary disease （COPD） and patients with COPD combined with diabetes mellitus. Methods A group of 22 cases with the COPD patients, B group of 21 eases with COPD combined with diabetic mellitus duration less than 10 years patients, C group of 18 cases with COPD combined with diabetes mellitus duration more than 10 years patients. All patients underwent lung function testing（ VC% ,FEV1% ,FVC% ,FEF 25% ～ 75% ,DLCO,DLCO/VA） and blood gas analysis （ PaO2, PaCO2 ）. Results The outcome of Group C VC% ,DLCO ,DLCO / VA,PaO2 were signifieantly lower than group A and B （ P 〈 0. 05 ）. Conclusion More advanced vital capacity decline and diffusing capacity impairments are found in COPD patients with type 2 diabetes mellitus dura- tion more than 10 years. Lung may be the target organ for diabetes mellitus.
