79例临床用药会诊的循证思维

Evidence-based Medicine Thoughts in Clinical Pharmacy Consultations of 79 cases

目的:总结临床药师进行用药会诊实践的具体案例,整理会诊过程中的临床循证思维,提高会诊的技能和思路。方法:回顾性分析2009—2011年上海市松江区中心医院临床药学室79例会诊案例情况,对科室分布、会诊要点、采纳情况、治疗效果进行统计,对每例会诊案例进行分析小结,归纳循证结论、循证思路。结果:申请用药会诊的科室主要分布在干部科、妇产科、骨科、血液科、综合重症监护室(GICU)、呼吸科;临床主要需求为抗菌药物治疗方案的制定;会诊意见被采纳的为76例,占96.3%;会诊后治疗预后为好转、痊愈出院的为63例,占79.7%;总结了进行用药会诊的循证思维。结论:临床药师进行用药会诊实践,并不断总结循证结论、循证思维,会大大提高自身的技能、拓宽思路;临床药师在临床药物治疗中发挥了重要的作用。

OBJECTIVE：To analyze the clinical pharmacy consultation cases in which there is involvement of clinical pharmacists,sorting evidence-based medicine thought in the consultation so as to improve the skill and thinking in clinical pharmacy consultation.METHODS： A total of 79 clinical pharmacy consultation cases in Shanghai Songjiang District Central Hospital from 2009 to 2011 was analyzed retrospectively with regard to distribution of clinical departments,key points in consultation,rate adoption of consultation opinions,and therapeutic efficacy.Each consultation case was analyzed and conclusions and thoughts of evidence-based medicine were summarized.RESULTS： The clinical pharmacy consultation was mainly applied for by departments of cadres,obstetrics gynecology,orthopedics,hematology,general intensive care unit and pneumology,and their main requirements focused on the formulation of antibacterial drug therapy regimen.Consultation opinions were adopted in 76 cases（96.3%）.63 patients（79.7%） improved or cured and discharged with treatment.The thoughts of evidence-based medicine were summarized.CONCLUSION： Clinical pharmacists＇ skill and thoughts were greatly improved through participating in clinical pharmacy consultation and summarizing the conclusions and thoughts of evidence-based medicine.Clinical pharmacists play an important role in clinical drug treatment.
