BiPAP无创呼吸机治疗慢性阻塞肺疾病的效果观察

Observation on the Effect of BiPAP Noninvasive Ventilator in the Treatment of Chronic Obstructive Pulmonary Disease

目的探讨BiPAP无创呼吸机治疗慢性阻塞肺疾病(COPD)的临床效果。方法选取2010年11月至2012年11月在我院治疗的150例慢性阻塞肺疾病患者的资料进行回顾性分析,分成两组,对照组予以常规的吸氧、抗感染、平喘祛痰等治疗,观察组在对照组基础上加用BiPAP无创呼吸机治疗,比较治疗后的效果情况。结果治疗前两组患者在PaO2、PaCO2、心率、呼吸、pH值方面的比较均无显著性差异(P>0.05),但是治疗后两组间有明显的差异性,特别是在治疗后的72h后差异明显(P<0.05),有统计学意义;对照组的平均住院时间高于观察组,在预后方面,在痊愈率、气管插管率、死亡率以及并发症的发生率上观察组优于对照组,两组比较有明显的差异(P<0.05),有统计学意义。结论 BiPAP无创呼吸机治疗慢性阻塞肺疾病对患者的肺功能有明显的改善作用,预后良好。

Objective To investigate the clinical effect of BiPAP noninvasive ventilator in the treatment of chronic obstructive pulmonary disease （COPD）. Methods The clinical data of 150 patients with chronic obstructive pulmonary disease from November 2010 to November 2012 in our hospital were retrospectively analyzed and divided into two groups. The control group was given with conventional oxygen, anti-infection and asthma expectorant treatment, the observation group was taken with BiPAP non-invasive treatment of ventilator treatment on the basis of the control group, to compare the effect after treatment. Results There was no significant difference in the PaO2, PaCO~, heart rate, breathing, PH value before treatment between the two groups （P 〉0.05）. There was significant difference after the treatment between the two groups, especially in the post-treatment 72 h, with significant difference （P〈0.05）. The average length of stay in the control group was higher than the observation group. In the prognosis, the effect of the incidence of cure rate, intubation rate, mortality and complications in the observation group was better than that in the control group. There was statistically significant difference between the two groups （P 〈0.05）. Conclusions BiPAP noninvasive ventilator m the treatment of chronic obstructive pulmonary disease has significant improvement in the lung function with better prognosis.
