无人操作防污染堵管吸痰器的研制及临床应用探讨

摘要 目的探索在气管切开管口连接"无人操作防污染堵管吸痰器"的临床应用价值。方法前瞻性收集2006年1月至2012年4月医院ICU脱机的气管切开患者64例为研究对象,分为A、B两组,每组32例,A组自愿选择使用"本吸痰器"自动吸痰,B组常规手动吸痰,观察两组RR、MAP、HR、血气变化及脱机后肺部感染、咳痰喷溅、痰栓痰阻、带管时间及再次上机率。结果 A组自动吸痰率87.5%,防咳痰喷溅和防污染堵管100%,A组内、两组间RR、MAP、HR、血气及再上机率比较P>0.05,两组肺部感染、痰阻及带管时间比较P<0.05。结论本吸痰器具有无人操作吸痰,防污染堵管,不影响BP、HR,不引起缺O2、PCO2蓄积及肺部感染等优点,可以安全用于气管切开自主呼吸患者的无人操作吸痰及堵管训练。 Objective To evaluate the application value of non-manual,pollution proof and duct blocking sputum ejector. Methods Six-four tracheotomy offline patients were chosen as study objects,with group A using this "sputum ejector" and group B routine sputum absorbing. RR, AMP, HR, PaO2, paCOz, online time, pulmonary infection rates and secondary respirator using rates between two groups were compared. Results RR, AMP, HR, PaO2, PaCO2, and secondary respirator using rates had no significant differences (P〉 0. 05), while pulmonary infection rates andical staffs' labor intensity. Online time did have significant differences (P〈0. 05). Conclusion This sputum ejector had advantages such aspreventing pulmonary infection,not affecting BP and HR, not reducing O2 and heotomy autonomous breathing patients. PCO2 storage. It could be safely applied to non-manual sputum ejection and duct blocking exercises for trac.
