选择性子宫动脉栓塞治疗产后出血45例临床分析

目的探讨选择性子宫动脉栓塞术治疗产后出血的疗效。方法 45例产后出血患者,经临床止血处理难于控制出血,急诊行选择性子宫动脉插管后,注入明胶海绵颗粒栓塞子宫动脉。结果双侧子宫动脉插管成功率100%,手术时间30～60min,术后所有患者出血停止,血压上升,心率下降,术后随访6～12个月,均无严重并发症出现,6例患者复孕。结论选择性动脉栓塞术创伤小,止血迅速有效,术后并发症少,对产后出血是一种安全有效的治疗措施。

Objective To investigate the efficacy of selective uterine artery embolization for postpartum hemorrhage. Methods 45 cases of postpartum hemorrhage were difficult to control bleeding by conservative treatment. With the help of femoral artery catheter, the gelatin sponge particles were injected into the uterine artery to control its bleeding. Results All cases of postpartum hemorrhage were hemostasised successful, the surgery time was 30 to 90 rain, the blood pressure （BP） increased and the heart rate （HR） decreased after selective embolization. All of the 45 cases had recovered well and 6 patients had been re-pregnant during a follow of 6 to 24 months. Conclusion Selective arterial embolization is minimally invasive, rapid and effective hemostasis, it is an effective and safe method for postpartum hemorrhage.
