脑动静脉周围组织慢性低灌注损伤及出血风险研究

目的探讨脑动静脉周围组织慢性低灌注损伤及出血的风险因素和临床策略。方法选取2009年1月～2010年12月收治的有脑动静脉周围组织慢性低灌注损伤及出血病史的56例患者,汇总临床资料与数据,分析风险因素,研究对策,观察临床效果。结果发生脑动静脉周围组织慢性低灌注损伤及出血风险的危险因素依次为:动脉硬化(73.21%)、BMI>25(60.71%)、血脂过高(53.57%)、年龄(21.42%)、不良生活习惯(14.28%)、其它(5.35%)。各因素间的比较差异有统计学意义(P<0.05或P<0.01);治疗8周后,患者的血压、胆固醇、血糖及并发症发生率分别为(124±10/78±8)mmHg(、4.25±2.72)mmol/L(、6.43±1.62)mmol/L、30.35%(17/56),较治疗前(136±8/88±6)mmHg(、6.23±3.84)mmol/L(、7.86±1.58)mmol/L、57.14%(32/56)均显著下降,比较差异均有统计学意义(P均<0.05)。结论积极控制脑动静脉周围组织慢性低灌注损伤及出血的风险因素,采取综合性的防治措施,有利于减缓局部脑组织低灌注损伤的病情。

Objective To discuss the clinical strategy of cerebral tissue around the vein chronic hypoperfusion injury and bleeding risk factors. Methods From 2009 January to 2010 December, 56 cases with tissue around the vein chronic hypoperfusion injury and 56 patients with cerebral hemorrhage history, summary of clinical data and data, analysis of risk factors, countermeasure research, clinical observation. Results The occurrence of cerebral tissue around the vein of chronic hypoperfusion injury and risk of bleeding risk factors were： arteriosclerosis （73.21%）, BMI 〈 25 （60.71%）, high blood fat （53.57%）, age （21.42%）, bad living habits （14.28%）, the other （5.35%）. There was significant difference between the factors （P〈0.05 or P〈0.01）; after 8 weeks of treatment, blood pressure, cholesterol, blood sugar and complication rate was （124± 10/78 ±8） mmHg, （ 4.25 ±2.72） mmol/L, （6.43 ±1.62） mmol/L, 30.35% （17/56）, a treatment before （136 ~ 8/88 ~6 ） mmHg, （6.23±3.84） retool/L, （7.86 ±1.58） mmol/L, 57.14% （32/56） were significantly decreased, there were significant differences （P 〈0.05）. Conclusion Positive control and risk factors of hemorrhage of the tissue around the vein chronic hypoperfusion injury cerebral, adopt comprehensive prevention and control measures, conducive to the mitigation of hypoperfusion injury cerebral disease.
