64层螺旋CT三维容积重建对肋骨骨折的诊断价值 被引量:49

摘要 目的比较X线片与64层螺旋CT三维容积重建(3DVR)图像在肋骨骨折诊断中的应用价值。资料与方法胸部外伤疑似肋骨骨折的100例患者,具有完整的普通X线片及64层螺旋CT轴位扫描图像与3DVR图像资料,比较X线片及3DVR图像对肋骨骨折的诊断敏感性和特异性。结果 100例患者中,CT轴位扫描图像诊断肋骨骨折91例共455处,3DVR图像诊断骨折89例共454处,X线片诊断骨折52例共206处。以CT轴位扫描图像为标准,3DVR图像诊断肋骨骨折的敏感性为97.80%,特异性为100.00%;X线片诊断的敏感性为60.46%,特异性为66.67%。结论 64层螺旋CT3DVR图像对肋骨骨折的诊断敏感性和特异性明显高于X线片,与CT轴位扫描图像相比,具有定位容易和骨折显示直观、明确等优势。 Purpose To compare the diagnostic value of three-dimensional volume rendering (3D VR) with X-ray film in rib fracture using 64-slice spiral CT. Materials and Methods 64-slice spiral CT and X-ray were performed in 100 patients with possible rib fracture after trauma, and 3D VR was applied with axial CT images. Sensitivity and specificity were calculated and compared between X-ray and CT. Results Ninety-one patients with 455 rib fractures were demonstrated based on axial CT images, 89 patients with 454 rib fractures based on 3D VR, and 52 patients with 206 rib fractures based on X-ray. The sensitivity and specificity of 3D VR were 97.80% and 100.00%, and those of X-ray were 60.46% and 66.67%. Conclusion 3D VR of 64-slice spiral CT has high sensitivity and specificity compared with X-ray film, and has the advantages of easy localization and easy display of the rib fracture.
