粘贴式负压冲吸器治疗四肢严重创伤的临床研究

Adhesive negative pressure suction apparatus in treating trauma to the extremities

目的探讨粘贴式负压冲吸器治疗军人四肢创伤的效果。方法对2011年5月至2012年5月期间收治的58例因军事训练造成四肢皮肤或软组织缺损的军人,采用粘贴式负压冲吸器持续冲吸治疗,对其疗效进行回顾性分析。结果 39例患者经持续冲吸治疗后创面肉芽组织生长新鲜,行二期缝合后创面修复成功,19例皮肤缺损严重者撤除装置后创面行网状皮片移植修复,14(73.7%)例创面愈合成功,5例创面皮片未存活或少量存活,经二次封闭式负压冲吸治疗再行网状皮片移植后创面修复成功。结论粘贴式负压冲吸器治疗能促进创面愈合,减少了换药次数,缩短了抗生素使用时间,具有较高的临床应用价值。

Objective To investigate the efficacy of adhesive negative pressure suction apparatus in treating trauma of the extremities of soldiers. Methods Fifty-eight soldiers with skin or soft tissue defects of the extremities caused by military training, were treated from May 2011 to May 2012 at the general surgery department of Fuzhou General Hospital of Nanjing Military Command. All of the patients were treated using the adhesive negative pressure suction apparatus, and their outcomes were analyzed retrospectively. Results After continuous treatment, 39 patients regained fresh granulation tissue and received secondary wound suture, and 19 patients with serious skin defects received wound mesh skin repair after removal of the device. Among the 19 patients, 14 （73.7%） had a successful wound healing and 5 patients had a wound healing after 2 cycles of treatment. Conclusion Application of adhesive negative pressure suction apparatus in the treatment of trauma of the extremities could be beneficial to the wound healing, reduce the number of dressing changes, and decrease the use of antibiotics.
