Radiotherapy of soft tissue sarcomas

外科手术在治疗软组织肉瘤中具有主导地位,以手术为主的辅助放射治疗是软组织肉瘤重要的综合治疗模式。术后辅助放疗对局部晚期软组织肉瘤能提高肿瘤局部控制率,延长生存期,已得到业界公认,应作为治疗常规,对复发病人术后放疗也应视为重要手段。近年来发展成熟起来的三维适形调强放疗新技术能最大限度地保护正常组织,减少治疗后的并发症,提高肿瘤控制率,应是今后的主流放疗技术。

Surgery remains the main treatment for patients with soft-tissue sarcoma, and every effort should be made to attempt resection. Postoperative external-beam radiation therapy is the first choice and remains the most widely practiced local adjuvant approach because it allows for sterilization of microscopic nests of residual disease without the need to postpone surgery. Postoperative radiotherapy to conservative surgery is shown in a randomized trial to enhance local control compared conservative surgery alone. Intensity-modulated radiation therapy in soft-tissue sarcoma provides excellent local control with a beneficiary effect in sparing normal tissues, which might be the main model of radiotherapy.
