彩色多普勒超声对闭合性肝破裂非手术治疗的应用分析

Application analysis of the color Doppler ultrasound with non-surgical treatment of the closed hepatorrhexis

目的探讨闭合性肝破裂的彩色多普勒超声分型对非手术治疗的应用指导价值。方法对43例单纯肝破裂患者彩色多普勒超声检查资料与临床治疗方案进行回顾性分析。结果彩色多普勒超声分型为I型19例（44．49％），Ⅱ型17例（39．53％），111型7例（16．28％）。43例患者35例非手术治疗成功，成功率为81．4％；中转手术治疗8例治愈。结论I型～Ⅱ型单纯性肝破裂患者可行非手术治疗，Ⅲ型患者可在彩色多普勒超声监测下动态观察病情进展、估计预后和调整治疗方案，也可达到良好疗效。

Objective To explore guidance value of the color Doppler ultrasound in non- surgical treatment to closed hepatorrhexis. Methods 43 cases of simple hepatorrhexis with color Doppler ultrasound screening and clinical treatment data were analyzed retrospectively. Results Application of the color Doppler ultrasound classified 19 type-I, 44.49%; 17 type-II, 39.53 %; 7 type-III, 16.28 %. 35 out of 43 cases were successful non-surgical treatments （81.4%）, while the rest eight cases were successful switched-back to surgery. Conclusion Non-surgical treatment is justified for type I and II hepatorrhexis, while for type III, the color Doppler ultrasound is suggested for monitoring and assessing the patient＇ s condition in order to make appropriate adjustment.
