血浆脑钠肽及Tei指数与妊娠高血压综合征患者关系的研究 被引量:6

摘要 目的探讨血浆脑钠肽(BNP)及Tei指数在妊娠高血压综合征患者(PIH)中的变化及其对心功能评价的价值。方法选择2010年7月~2012年7月广东医学院附属厚街医院就诊的孕龄28~40周的PIH孕妇50例(研究组)及同期孕龄28~40周健康孕妇40例(对照组),应用ELISA方法检测血浆BNP浓度;采用超声心动图记录两组二、三尖瓣环的运动频谱,测量组织多普勒时间间期,计算组织多普勒Tei指数,比较两组Tei指数有无差异;比较研究组左、右心室Tei指数有无差异。结果①研究组与对照组血浆BNP含量[(123.88±58.69)pg/mL、(35.89±11.52)pg/mL]及左心室Tei指数[(0.590±0.067)、(0.456±0.073)]、右心室Tei指数[(0.621±0.090)、(0.475±0.090)]比较,差异均有高度统计学意义(均P<0.01)。②研究组左心室Tei指数为(0.590±0.067),右心室Tei指数为(0.621±0.090),两者比较,差异无统计学意义(P=0.132>0.05)。结论血浆BNP水平及Tei指数是评价PIH患者心功能的有效、敏感指标,PIH患者左右心室功能损害程度无明显差异。 Objective To study the changes of plasma concentration of BNP and Tei index and the evaluation of cardiac function in patients with pregnancy-induced hypertension (PIH). Methods 50 cases of PIH pregnant woman (gestational age of 28-40 weeks) from July 2010 to July 2012 in Houjie Affiliated Hospital of Guangdong Medical College were selected as experiment group, 40 cases of healthy pregnant women (gestational age of 28-40 weeks) in the same term were selected as control group. ELISA method was used to detect the plasma concentration of BNP in the two groups. Tissue Doppler was used to detect the dynamic spectrum of valvula bicuspidalis and tricuspid annulus, tissue Doppler time interphase was measured, Tei index was calculated and compared in the two groups; Tei index of ventriculus sinister and ventriculus dexter in experiment group were compared. Results ①The difference of plasma concentration of BNP in experiment group [(123.88±58.69) pg/mL] and control group [(35.89±11.52) pg/mL] was statistically significant (P 〈 0.01); the difference of Tei index in ventriculus sinister in experiment group (0.590±0.067) and control group (0.456±0.073) was statistically significant (P 〈 0.01); the difference of Tei index in ventriculus dexter in experiment group (0.621±0.090) and control group (0.475±0.090) was statistically significant (P 〈 0.01). ②Tei index in ventriculus sinister in experiment group was (0.590±0.067), Tei index in ventriculus dexter in experiment group was (0.621±0.090), the difference of Tei index in ventriculus sinister and ventriculus dexter in experiment group was not statistically significant (P = 0.132 〉 0.05). Conclusion The plasma concentration of BNP and Tei index are sensitive indexes of heart function with pregnancy-induced hypertension. There are no difference between left and right ventricular damage of patients with PIH.
