胃充盈超声在胃癌TNM分期中的应用研究

目的探讨胃充盈超声造影在胃癌TNM分期中的应用价值。方法54例胃癌术前患者进行胃超声造影检查及TNM分期，并与术后结果、病理对照分析结果超声对胃癌浸润深度T1、T2、T3和T4诊断的符合率分别为40％、46.2％、76．0％、81．8％，总符合率为66．7％；对淋巴结转移N0,N1，N2，N3期诊断符合率分别为86．4％，61．1％，54．6％，33．3％，对肝脏、腹膜转移的诊断符合率为100％。结论胃充盈超声对胃癌具有较高的临床分期符合率，可作为胃癌术前常规检查手段，但其对于早期胃癌的T分期诊断符合率低．对胃大弯、胃底部肿痛易漏诊。

Objective To investigate the value of enntrast-enhaneed uhrasnno- graphy （CEUS） in gastric carcinoma TNM staging. Methods Fifty four patients underwent contrast uhrasonography,and the results were compared with the pathological finding. Results The delceting accuracy of the cancer invasion depths （T1,T2,T3,T4,were40%,46.2%.76.0%,81.8%,respectiveing and the whole accuraev was 66.7%）,The whole accuracy for N staging （N0,N1,N2,N3 were 86.4%,61.1%,54.6%,33.3eA）.The accuracy for detemlining distant metastases was lO0%.Conclusion It has high accuracy for advanced staging of the CEUS in diagnosing gastric earcinoraa.The limitation of CEUS was the difficulty of diagnosis of eareinomus in fundns and gastric curvature easily to be missed diagnosed and of low accuracy for early T stage of the carcinoma.
