痰热清注射液治疗重症肺炎痰热壅肺证临床疗效观察

Clinical Observation of Tanreqing Injection in Treatment of Severe Pneumonia of Phlegm Obstructing Lung

目的:观察痰热清注射液治疗重症肺炎痰热壅肺证的临床疗效。方法:将2009年1月—2012年4月入中国中医科学院西苑医院ICU内的108例重症肺炎痰热壅肺证的患者随机分为两组,对照组采用西医常规综合治疗,治疗组在对照组的基础上加用痰热清注射液40 mL,1日1次静脉滴注14 d。比较两组临床疗效,具体观察指标包括:体温(T)、呼吸频率(RR)、白细胞计数(WBC)、动脉血氧分压(PaO2)、氧合指数(PaO2/FiO2)、CPIS评分、机械通气时间(h)以及ICU住院天数。结果:治疗组总有效率高于对照组,ICU住院时间短于对照组,临床观察指标改善情况优于对照组,差异均有统计学意义(P<0.05)。结论:痰热清注射液具有良好的清热、化痰、抗炎症作用,对于重症肺炎痰热壅肺证的治疗发挥着积极作用。

Objective ： To observe the clinical efficacy of Tanreqing Injection in the treatment of severe pneumonia of phlegm obstructing hmg. Method： 108 cases of severe pneumonia patients with phlegm obstructing lung in January 2009 to April 2012 in Department of Critical Care Medicine, Xiyuan Hospital, China Academy of Chinese Medical Sciences, were randomly divided into two groups, the control group was treated with the conventional Western therapy, the treatment group added Tanreqing Injection intravenously at a dose of 40 mL, once a day, for 14 d, on the basis of the control group. The clinical efficacy and specific observations indicators including body temperature（ T ）, respiratory rate（ RR ）, white blood cell count（ WBC ）, arterial oxygen pressure （ PaO2 ）, oxygenation index （ PaO2/FiO2 ）, CPIS score, mechanical ventilation time （ h ）, and ICU length of stay were compared. Results ： The total efficiency of the treatment group was higher than that in control group and ICU length of stay was shorter than that of the control group, the clinical observation indicators to improve the situation were better than those of the control group, the differences were statistically significant （ P〈0.05 ）. Conclusion ： Tanreqing Injection has good heat-clearing, phlegm- resolving, anti-inflammatory effect and plays an active role in treatment for severe pneumonia phlegm obstrucing lung.
