1%聚维酮碘液龈下冲洗对牙周炎的治疗作用 被引量:3

摘要 目的观察龈下超声刮治及根面平整后,采用1%聚维酮碘进行牙周袋内冲洗治疗慢性牙周炎的疗效。方法选择中度和重度慢性牙周炎患者30人,进行全口洁治、龈下超声刮治及根面平整,选取左侧前牙较重的一颗为实验组,用1%的聚维酮碘液进行牙周袋内冲洗;对照组选用同患者对侧同名牙,用生理盐水冲洗;避免选相邻的牙。记录治疗前、治疗后3、6个月的菌斑指数、探诊出血、牙周袋深度、附着丧失、龈沟出血指数;及厌氧菌和产黑色素类杆菌总数。结果治疗3个月后,2组的牙周袋探诊深度、菌斑指数、龈沟出血指数及探诊出血较治疗前明显改善(P<0.05);实验组附着丧失比治疗前明显下降(P<0.05);实验组牙周袋探诊深度、附着丧失、龈沟出血指数及探诊出血改善程度均大于对照组(P<0.05);2组牙周袋内厌氧菌总数与产黑色素类杆菌量较治疗前均有降低(P<0.05),实验组优于对照组(P<0.05)。治疗6个月后与治疗3个月后各项临床指标及细菌学检查结果基本相同,差异无统计学意义(P>0.05)。结论龈下超声刮治及根面平整结合1%的聚维酮碘液牙周袋内冲洗,能提高治疗效果及较长时间控制龈下菌斑。 Objective To evaluate the effect of ultrasonic scaling and root planning with synchronous 1% Povidone-iodine(Pvp-I) irrigation on periodontitis. Methods Thirty patients with moderate or severe periodontitis for supragingival scaling, ultrasonic scaling and root planning were included. After scaling, one side of the front teeth was rinsed with 1% Pvp-I(test group), and the opposite side of the front teeth washed with 0. 9% normal saline (control group). Before and after treatment for 3 and 6 months, plaque index ( PLI), bleeding on probing ( BOP), pocket depth ( PD), attachment level( AL), sulcus bleeding index( SBI), anaerobic bacteria and prodution of black pigmented bacteroides were recorded. Results Three months after treatment, the pocket depth ( PD), plaque index ( PLI), sulcus bleeding index (SBI) and bleeding on probing (BOP) significantly improved in both groups ( P 〈 0. 05 ). Attachment level (AL) in test group decreased(P 〈 O. 05 ). The improvement in pocket depth ( PD), attachment level ( AL), sulcus bleeding index ( SBI ) and bleeding on probing(BOP) in test group was more significant than in the contral (P 〈 0. 05 ). The quantity of anaerobic bacteria and production of black pigmented bacteroides were lower in test group than in the control ( P 〈 0. 05 ). All the chnical indexes 6 months after treatment were not significantly different from those of 3 months ( P 〉 O. 05 ). Condusion 1% Pvp-I as irrigation solution can enhance the ultrasonic scaling and root planning effect in the treatment of chronic periodontitis
