肾上腺海绵状淋巴管瘤3例临床病理观察

Adrenal gland cavernous lymphangioma:a clinicopathologic analysis of three cases

目的探讨肾上腺海绵状淋巴管瘤的病理学特征。方法收集3例肾上腺海绵状淋巴管瘤的临床病理资料并随访,所有标本均经HE染色、光镜观察和免疫组化染色。结果 3例均为男性,年龄30～75岁,平均51岁。光镜下瘤组织由大小不等的腔隙组成,腔内壁衬以单层扁平内皮细胞,腔内含有少数淋巴细胞,可见红细胞,腔隙之间的间质由胶原纤维组成,并见灶性淋巴细胞聚集。免疫组化:瘤细胞D2-40部分(+),CD31、CD34、EMA、SMA、CK5/6和calretinin均为(-)。结论肾上腺海绵状淋巴管瘤属淋巴系统的良性肿瘤,其特征性的组织形态、免疫组化表型有助于诊断和鉴别诊断。

Objective To study the pathological characteristic of adrenal gland cavernous lymphangioma. Methods Clinical data were collected in ihree cases of adrenal gland cavernous lymphangioma with follow-up. All the samples were hematoxylin and eosin stained, which were also subjected to immunohistochemical stain by multiple antibodies, including D2-40, CD31, CD34, EMA, SMA, CK5/6 and calretinin. Results Three samples were all from male, and their age was 30 years to 75 years, with average of 51 years. The tumor was composed of irregular shaped lacune under light microscope, endosporium was monostratified of fiat endothelia. There were little lymphoeytes and erythrocytes within it. Mesenehymal collagen fiber and also lymphocytes aggregation were noted between lacune. Immunohistochemically, D2-40 was partly expressed, and CD31, CD34, EMA, SMA, CK5/6 and calretinin were negative. Conclusion Adrenal gland cavernous lymphangioma is a benign neoplasm of lymphatic system, and its distinctive organization appearance and immunohistochemical phenotype are useful for diagnosis and differential diagnosis.
