摘要
目的 探讨胆胰管结扎诱发胆源性胰腺炎 (Bp)的发生发展过程中 ,血浆生长抑素 (SS)含量的变化和其变化的机制。方法 分别观察了SD大鼠胆胰管末端结扎组 (A组 ,18只 )、胆总管结扎组 (B组 ,18只 )及静脉推注胆囊收缩素八肽 (CCK OP)组 (C组 ,18只 ,每次 30 μg/kg体重 ,1次 /h ,共 3次 ) ,不同时期血浆生长抑素八肽 (SS OP)及血浆淀粉酶含量和胰腺的病理变化。结果 A、C组分别在管腔结扎及静脉输注CCK OP后 3h ,血浆SS OP及淀粉酶含量均明显高于B组及对照组 (D组 ,18只 ,P <0 .0 5 ) ;在 6h及 2 4h ,A、C组血浆SS OP均降至正常 ,血浆淀粉酶仍明显高于D组 (P <0 .0 5 )。B组在管腔结扎后 3h ,血浆SS OP含量明显高于D组 (P <0 .0 5 ) ,但明显低于A、C组 (P <0 .0 5 ) ;B、D组血浆淀粉酶在各个观察点均无明显升高。A、C组在各个观察点均可见胰腺炎性水肿 ,B、D组胰腺均无异常。因此 ,胆胰管结扎和胆总管结扎后均可使血浆SS OP含量升高 ,但升高的幅度差异有显著性 ;静注CCK OP后也可使血浆SS OP及淀粉酶含量升高和致胰腺炎性水肿变化。结论 胆胰管结扎诱发Bp过程中 ,血浆SS OP发生的明显升降变化与CCK OP的变化有关。
Objective\ To investigate the alternation and mechanism of the change in plasma somatostatin octapeptide (SS OP) level during biliary pancreatitis induced by bile pancreatic ligation. Methods\ The contents of plasma SS OP, blood amylase and the changes of pancreatic morphology of the rats in group A ( n =18, ligating bile pancreatic duct), B ( n =18, ligating bile duct), C [ n =18, infusing cholecystokinin (CCK) OP intravenously 3 times with 30 μg/kg each time] and the changes of pancreatic morphology of rats in groups A, B, C and D ( n =18, sham operation, dissection bile pancreatic duct, no ligation). Results\ The contents of plasma SS OP and blood amylase were increased rapidly in groups A and C 3 h after experiment. At 6th h and 24th h, plasma SS OP level returned to normal,blood amylase levels in groups A and C were still higher than in the groups B and D ( P <0.05). Three h after duct ligation, plasma SS OP content in the group B was obviously higher than in the group D ( P <0.05), but significantly lower than in the groups A and C. Morphological changes of acute pancreatitis in the groups A and C were found at 3, 6 h and 24 h, but pancreases were normal in the groups C and D. Conclusion\ It was suggested that the changes in plasma SS OP during biliary pancreatitis induced by pancreatic bile duct ligation were related with the changes of CCK OP.\;
