结节性类天疱疮1例

A Case of Nodular Pemphigoid

患者男,63岁。全身结节、水疱伴剧烈瘙痒半年。皮肤科情况:躯干、四肢泛发褐色结节及苔藓化损害,部分表面浅溃疡、结痂及色素减退,双上臂少数张力性水疱。左上臂皮损组织病理示:表皮下水疱形成,疱内大量嗜酸性细胞,真皮浅层血管周围嗜酸性细胞及少量淋巴细胞浸润。直接免疫荧光检查显示:IgG和C3沿皮肤基底膜带呈线状沉积。诊断:结节性类天疱疮。

A 63-year-old male presented with widespread nodules and blisters with intense pruitus for half years. Brown nodules and liehenified lesions were distributed on his trunk and limbs diffusely. Superficial ulcers, crusts and hypopigmentation occurred in some lesions. A few tense vesicles were also found in his upper arm. The histopathology, of a biopsy from his left upper arm showed that a subepidermal bulla with numerous ensinophils, and a perivaseular infiltration of eosinophils and lymphocyles in the upper dermis, Direct immunourescence demonstrated linear deposition of IgG and C3 at the basement membrane zo,le. The diagnosis of nodular pemphigoid was established.
