胃十二指肠动脉狭窄闭塞及侧支循环的MSCTA表现

Multi-slice CT Angiography Manifestations of Stenosis or Occlusion and the Collateral Circulation of Gastroduodenal Artery

目的探讨胃十二指肠动脉(GDA)狭窄闭塞及侧支循环在多层螺旋CT血管造影(MSCTA)中的表现。方法搜集行MSCTA检查诊断为GDA狭窄闭塞的患者22例,采用容积再现重组(VR)血管生长技术(AV)进行血管重建。结果 22例患者中,GDA狭窄10例,4例多发性狭窄均显示前、后动脉弓扩张,6例局限性狭窄均显示前动脉弓扩张。GDA闭塞12例,均显示胃网膜右动脉直接与前动脉弓连接,6例全段闭塞者胰十二指肠上后动脉(PSPDA)不显影,前动脉弓扩张,6例局限性闭塞均发生于GDA远段,4例前动脉弓扩张,2例前、后动脉弓扩张。结论 MSCTA能清楚显示GDA狭窄闭塞及侧支循环情况。

Objective To explore the multi-slice spiral CT angiography （MSCTA） manifes- tation of stenosis/occlusion and the collateral circulation of gastroduodenal artery （GDA）. Methods 22 patients with stenosis or occlusion of gastroduodenal artery underwent MSCTA examination, and the blood vessels were reconstructed with the post-processing techniques including volume rendering （VR） or acid vessel （AV）. Results Of the 22 patients, there were occlusion （12 cases） of GDA, all shown the right gastroepiploic artery communicated with the anterior arterial arcade directly. The posterior superior pancreaticoduodenal artery was not displayed and the anterior arcade was expanded in the GDA occlusion completely （6 cases）. Local occlusion of distal GDA were seen in 6 cases, the anterior arterial expansion were seen in 4 cases, anterior and posterior arterial expansion were seen in 2 cases. Conclu- sion The situation of collateral circulation as well as stenosis/occlusion of the GDA could be clearly displayed on MSCTA reconstruction images.
