中性粒细胞相关载脂蛋白在感染幽门螺杆菌的胃黏膜中的表达情况及其临床意义

Expression of neutrophil gelatinase-associated lipocalin in gastric mucos infected with Helicobacter pylori and its clinical significance

目的研究中性粒细胞相关载脂蛋白(NGAL)在感染幽门螺杆菌(Hp)胃组织中的表达及其临床意义。方法对8例胃癌手术切除标本进行逆转录聚合酶联反应(RT-PCR),检测NGAL mRNA相对表达量;应用Warthin-Starry银染色法和Gramse染色法在胃黏膜中查找Hp;采用免疫组织化学SP方法检测NGAL蛋白在11例正常胃黏膜、12例慢性萎缩性胃炎(伴肠化)、27例异型增生(轻、中、重度各9例)、10例早期胃癌以及30例进展期胃癌(高、中、低分化各10例)中的表达情况。结果 90例胃组织中有59例NGAL表达阳性;正常胃黏膜,肠上皮化生,异性增生,早期胃癌与进展癌胃癌之间NGAL蛋白表达强度的差异具有统计学意义(P=0.029),而不同分化程度的进展期胃癌组织之间NGAL蛋白表达差异无统计学意义。感染Hp的胃组织(n=59)与未感染Hp的胃组织(n=31),NGAL蛋白表达强度差异具有统计学意义(P=0.022)。结论胃癌组织中NGAL的过度表达,表明该基因的表达上调可能与胃癌的发生有关;感染Hp的胃组织中NGAL表达明显增高,表明NGAL的增高与Hp感染有关,两者之间可能有共同促进胃癌发展的作用;NGAL蛋白的过度表达可能是人胃癌早期诊断、发生发展以及判断预后的潜在生物学参考指标。

Objective To study the neutrophil gelatinase-associated lipocalin（NGAL）expression and its clinical significance in gastric mucosa infected with Helicobacter pylori（Hp）.Methods Eight cases of human gastric cancer specimens were collected from patients who underwent surgical treatment.Reverse transcription-polymerase chain reaction（PT-PCR）was used to measure the expression of NGAL mRNA.Warthin starry silver staining and Giemsa staining were used to detect the Hp.SP immunohistochemical method was employed to detect the expression of NGAL in different stages of pre-neoplastic leading to gastric cancer.Results Immunohistochemical staining results showed that positive NGAL expression in the gastric tissues was 65.56%and the high NGAL expression was showed in intestinal metaplasia,dysplasia,and gastric cancer （P=0.029）.In gastric cancer lesions,NGAL staining was found with similar frequency in differentiation degree（P=0.892）.The whole gastric tissues,Hp-positive cases（n=59）were found more frequently than noninfected cases（n=31,P=0.022）.Conclusion The over-expression of NGAL may correlate with early diagnosis,the carcinogenesis and prognosis of gastric cancer.
