联合用药治疗老年单纯收缩期高血压的疗效观察

A curative effect observation of combination therapy of elderly patients with isolated systolic hypertension

目的比较单硝酸异山梨酯片联合苯磺酸氨氯地平片和福辛普利片治疗老年单纯收缩期高血压(isolated sys-tolic hypertension,ISH)的疗效。方法 68例老年ISH患者中34例口服单硝酸异山梨酯片、苯磺酸氨氯地平片和福辛普利片治疗(治疗组),另34例口服苯磺酸氨氯地平片和福辛普利片治疗(对照组),疗程均为8周。比较治疗第1、5、8周的脉压(pulse pressure,PP)、收缩压(systolic blood pressure,SBP)及舒张压(diastolic blood pressure,DBP),疗程结束后比较两组的疗效。结果治疗第1周,治疗组SBP及PP下降幅度均大于对照组,DBP下降幅度小于对照组,且随着疗程的延长,治疗组SBP及PP先于对照组在第5周降至正常,两组比较,差异均有统计学意义(P<0.05)。疗程结束后,两组总有效率比较差异有统计学意义(χ2=6.324,P<0.05)。结论单硝酸异山梨酯片联合苯磺酸氨氯地平片和福辛普利片治疗老年ISH总有效率明显优于苯磺酸氨氯地平片与福辛普利片,不良反应少,是治疗老年ISH的有效方案。

Objective To observe the clinical effect of combination of isosorbide mononitrate tablets with amlodipine besylate tablets and fosinopril tablets in treatment of elderly patients with isolated systolic hypertension （ISH）. Methods Sixty-eight patients with ISH in our hospital were retrospectively analyzed. Of the patients,34 were treated with the three medicine （ treatment group） while 34 patients were treated with amlodipine besylate tablets and fosinopril tablets （control group） for 8 weeks. Results After treatment, there was significant difference in curative effect between the two groups（P 〈 0.05 ）. The total effective rate was 85.29% in the treat- ment group while 67.64% in the control group. No specific adverse reactions were found in the two groups during the treatment process and after treatment. Conclusions The combination of the three medicines in the treatment of elderly patients with ISH had better effects than that of combination of amlodipine besylate tablets and fosinopril tablets with fewer side effects. Therefore, the therapeutic regimen is suitable for elderly patients with ISH.
