糖尿病肾病患者用药依从性的质性研究

Adherence to Prescribed Medications in Patients with Diabetic Kidney Disease: A Qualitative Study

目的探讨影响糖尿病肾病患者用药依从性的因素。方法对14例糖尿病肾病患者进行半结构式深度访谈,应用Johnson用药依从性模型对资料进行分析。结果糖尿病肾病患者对所用药物的必要性、有效性和安全性存在不同程度的不确定感;患者在药物获取、行为习惯和记住用药等方面遇到的问题常导致非故意不遵医行为的出现。结论医务人员应充分认识阻碍糖尿病肾病患者遵医用药的因素,通过加强专科间沟通协作、提高治疗方案合理性、加强随访和针对性健康指导、促使家属积极参与等措施,提高患者的用药依从性,改善疾病预后。

Objective To discuss the factors affecting adherence to prescribed medications for patients with diabetic kidney disease. Methods In-depth semi-structured interviews were conducted to 14 patients with diabetic kidney disease. The data were analyzed based on Johnson＇s model of medication adherence. Results Patients were not convinced of the need, effectiveness and safety of their medications. Problems in medication access, routine and remembering contributed to patients＇ unintentional non-adherence. Conclusion Healthcare professionals should acknowledge the barriers as perceived by patients with diabet- ic kidney disease, and elevate medication adherence and improve the prognosis of the disease by developing specialized interventions including strengthening communication and cooperation between specialists, im- proving the rationality of the treatment program, strengthening the follow-up and targeted health guidance and prompting families to actively participate in the treatment.
