磁共振水激励三维扰相梯度回波序列在膝关节软骨损伤诊断中的临床研究 被引量:5

摘要 目的探讨磁共振水激励三维扰相梯度回波序列(3D-WATSc)在膝关节软骨损伤诊断中的临床应用价值。方法收集经3D-WATSc序列检查和膝关节镜检查的80例膝关节受检病例,评估400个膝关节软骨面,分析2种检查方法的敏感度、特异度和准确度,一致性指标采用Kappa值。结果 3D-WATSc扫描序列检查的诊断敏感度为92.0%,特异度为90.6%,准确度为91.8%,0级Kappa值0.729、Ⅰ级Kappa值0.703、Ⅱ级Kappa值0.624、Ⅲ级Kappa值0.787、Ⅳ级Kappa值0.899。结论 3D-WATSc扫描序列与膝关节镜检查之间有较高的一致性,是膝关节软骨损伤的首选检查方法。 Objective To investigate clinical value of water excitation three-dimensional spoiled gradient echo sequence (3D-WATSc) in the diagnosis of knee cartilage injury. Methods Collected 80 patients with knee joint which were examined by 3D-WATSc and arthroscopy, assessment of the 400 cases of knee cartilage surface, analysis of two methods of sensitivity, specificity, accuracy, consistency index using Kappa value, Result The diagnostic sensitivity of the 3D-WATSc scan sequence was 92.0%, the specificity was 90.6%, and the accuracy was 91.$%, 0 grade Kappa value was 0.729, Ⅰ grade Kappa value was 0303, Ⅱgrade Kappa value was 0.624, Ⅲgrade Kappa value was 0.797, Ⅳgrade Kappa value was 0.899. Conclusion 3D-WATSc and arthmscopy have higher consistency, which is the preferred examination of knee cartilage injury.
