老年糖尿病低血糖昏迷35例临床分析

Clinical Analysis of 35 Elderly Diabetes Mellitus Patients with Hypoglycemic Coma

目的：探讨老年糖尿病低血糖昏迷的临床治疗方法及疗效。方法：对我院2008年9月至2011年10月收治的35例老年糖尿病低血糖昏迷患者的临床疗效进行分析。结果：经治疗后,32例患者的意识得到恢复,肢体恢复正常活动。患者苏醒时间（0.5-18）h,中位苏醒时间为1.2h。治疗后空腹血糖（5.3-7.5）mmol/L,平均（5.9±0.6）mmol/L。治疗后患者的血糖水平显著升高,差异有统计学意义（P〈0.05）。有3例发生脑梗死,其中1例因昏迷时间过长,脑细胞严重受损,继而发生呼吸衰竭而死亡。结论：应高度重视老年糖尿病低血糖昏迷的危害性,采用恰当的药物进行治疗,加强对老年糖尿病患者的健康教育,以减少低血糖昏迷的发生。

Objective：To explore the clinical treatment method and effect of senile diabetes mellitus with hypoglycemic coma.Methods：Clinical effect of 35 cases of senile diabetes mellitus with hypoglycemic coma treated in our hospital from September 2008 to October 2011 were analyzed.Results：after the treatment,32 patients＇ consciousness were recovered,and normal activities of limb resumed.The recovery time of patient was（0.5~18）h,and the median recovery time was 1.2h.The fasting blood glucoses after treatment were（5.3~7.5）mmol/L,and the average was（5.9 ＋0.6） mmol/L.The blood glucose levels after treatment increased significantly,and the difference was statistically significant（P0.05）.There were 3 cases with cerebral infarction,including 1 cases dief of respiratory failure because of long time of coma and heavy damage of brain cells.Conclusion：great importance should be attached to the harm of senile diabetes mellitus with hypoglycemic coma.We should adopt appropriate drug treatment,strengthen the health education of elderly diabetes patients in order to reduce the occurrence of hypoglycemia coma.
