小剂量胺碘酮治疗快速室上性心律失常的疗效分析

Efficacy Analysis of Low-Dose Amiodarone in Treatment of Fast Supraventricular Arrhythmia

目的：探讨小剂量胺碘酮治疗快速室上性心律失常的临床疗效和不良反应。方法：将我院救治的发作时间〈48h,病程〈6个月的快速室上性心律失常患者84例随机分为两组,每组42例,大剂量组给予300mg胺碘酮加入5%葡萄糖50ml,静脉推注后以1mg/min静脉点滴6h,0.5mg/min维持点滴（12~36）h;小剂量组给予150 mg胺碘酮加入5%葡萄糖50ml,静脉推注后以1mg/min静脉点滴6h,0.5mg/min维持点滴（12~36）h,观察其临床疗效和不良反应。结果：大剂量组1h内转复4例（9.5%）,8h转复22例（52.4%）,12h转复36例（85.7%）,总有效率为85.7%;小剂量组1h内转复2例（4.7%）,8h转复19例（45.2%）,12h转复34例（81.0%）,总有效率为81.0%。两组转复率比较无统计学差异（P〈0.05）。两组患者治疗期间未见严重不良反应。结论：小剂量胺碘酮治疗快速室上性心律失常,选择适当病人转复率较高,疗效相同,但小剂量用药安全性更高,建议使用。

Objective： To investigate the efficacy and adverse reactions of small-dose amiodarone treatment of rapid supraventricular arrhythmia.Methods：84 of rapid supraventricular arrhythmia patients which duration 6 months,The onset time of our hospital 48h,were randomly divided into two groups,each group 42 cases,large-dose group were given 300mg of amiodarone in 5% glucose 50ml after intravenous injection to 1mg/min intravenous drip 6h 0.5 mg/min maintain drip（12~36）h;low-dose group given 150 mg amiodarone by adding 5% glucose 50ml intravenous injection to 1mg/min intravenous drip after 6h,0.5mg/min maintain drip（12~36）h.Results：highdose group within one hour cardioversion was 4 cases（9.5%）,8h cardioversion was 22 cases（52.4%）,12h turn was 36 cases（85.7%）,total effective rate was 85.7%;cardioversion within one hour of low-dose group was 2 cases（4.7%）,8h cardioversion was 19 cases（45.2%）,12h cardioversion was 34 cases（81.0%）,the total effective rate was 81.0%.Two cardioversion rate was no significant difference（P 0.05）.The two groups of patients during treatment were no serious adverse reactions.Conclusion： small doses of amiodarone treatment of rapid supraventricular arrhythmia,select the appropriate transfer of patients to the complex higher rate of efficacy of the same,but small doses of medication and more secure,we recommend to use.
