加强围术期健康宣教对前庭神经鞘膜瘤患者治疗效果的影响 被引量:2

摘要 目的探讨加强围术期健康宣教对前庭神经鞘膜瘤患者治疗效果的影响。方法 2007年1月至2009年12月,便利抽样法选择上海交通大学医学院附属新华医院耳鼻咽喉-头颈外科就诊的前庭神经鞘膜瘤患者96例为研究对象,以2007年1月至2008年8月的48例患者为对照组,以2008年9月至2009年12月的48例患者为观察组。对照组患者给予常规护理,观察组患者加强围术期健康宣教,分别于患者入院当天及出院前1d采用焦虑自评量表(self-rating anxiety scale,SAS)、抑郁自评量表(self-rating depression scale,SDS)及自制调查表对其进行全面评估,比较两组患者干预前后SAS评分情况及DI,并观察患者对治疗及护理的态度及并发症发生情况。结果两组患者宣教前后的SAS评分、DI的差异有统计学意义(均P<0.05)。观察组患者SAS评分及DI均优于对照组,差异均有统计学意义(均P<0.01),且观察组患者对治疗及护理的态度优于对照组,且术后并发症低,差异有统计学意义(P<0.01)。结论加强围术期健康宣教可以明显改善前庭神经鞘膜瘤患者的心理状态和生活质量,减少手术并发症,提高治疗和护理的依从性。 Objective To investigate the therapeutie effect psychological influence of strengthened perioperative health education on vestibular schwannoma patients. Methods Using convenience sampling, 96 patients with ves- tibular schwannoma who received treatment of otolaryngology-head neck surgery in our hospital were enrolled in the study. Forty-eight patients between January 2007 and August 2008 were selected as control group and 48 patients between September 2008 and December 2009 were taken as intervention group. The control group was given routine nursing care and the intervention group received strengthened health education during the periopera- tive period. On the admission day and the day before discharging from hospital, the self-rating anxiety scale(SAS), self-rating depression scale(SDS) and self-made questionnaire were adopted to evaluate the patients comprehensive- ly. The patients' SAS scores and DI before and after the intervention were compared, and the patients' attitude to- wards the treatment and nursing and incidence of complications were observed of the two group. Results The differences of SAS scores and DI of the two groups before and after health education had statistical significance(P〈 0.05). The SAS scores and DI of the intervention group were significantly higher than those of the control group (P〈0.01). Compared with the control group, the attitude towards the treatment and nursing of intervention group patients was significantly better and the rate of postoperative complications was lower (P〈O. 01). Conclusion Strengthened peripoerative health education can obviously improve vestibular schwannoma patients' psychological state and the quality of life, reduce the complications and improve the treatment and nursing compliance.
