顺腋皱襞小切口大汗腺摘除术根治腋臭

Orthophoric Apocrine Gland Extraction with Small Incision Along Axillary Fold for Axillary Osmidrosis

目的探讨腋臭根治的理想术式。方法采用顺腋皱襞小切口，翻转局部皮肤，直视下行大汗腺、毛囊摘除术，保留真皮下血管网组织及表面完整的皮肤组织。结果本组198例患者均未出现上臂皮肤麻木及腋下皮肤紧缩感，术后均无伤口感染和皮肤坏死。术后有8例出现皮下小血肿，经清创换药后愈合。本组中治愈172例（87％）；有效26例（13％）。术后随访3～24个月，腋下瘢痕均不明显，无明显异味，效果满意。结论顺腋皱襞小切口大汗腺摘除术，具有术后恢复快、效果好、复发率低、癜痕不明显等优点，是治疗腋臭的理想方法。

Objective To investigate the ideal surgical method for axillary osmidrosis. Methods From June 2008 to October 2010, 198 cases with axillary osmidrosis were treated by orthophoric apocrine gland extraction with small incision along axillary fbld under local anesthesia. The apocrine glands and folliculus pili were excised but subcutaneous capillary network, surface skin were remained. Results No upper arm skin numbness, alar skin tightening, infection and necrosis were observed except 8 cases with subcutaneous hematoma but recovered by dressing. In 198 cases, 172 cases were cured （87%） and 26 cases were effectively treated （13%）. All the cases were followed up for 3 to 24 months, no obvious scar and foreign flavor were observed. Conclusion Orthophoric apocrine gland extraction with small incision along axillary fold has many advantages, such as shorter postoperative recovery, high efficiency, low rate of recurrence, no obvious scar and so on, and is considered as an ideal method for the treatment of axillary osmidrosis.
