会阴无保护接生技术在低危孕妇正常分娩中的应用

Application of Unprotected Perineum Delivery Technique of Low Risk Pregnant Women

目的探讨会阴无保护接生技术在低危孕妇正常分娩中的应用效果。方法便利抽样法选择2012年1-6月在厦门市妇幼保健院拟经阴道分娩的初产妇400例为研究对象。按随机数字表法将其分为观察组和对照组,每组200例。观察组产妇采用会阴无保护接生技术,对照组产妇采用常规接生技术。比较两组产妇会阴切开情况、会阴裂伤程度、第二产程时间及产程中疼痛情况。结果观察组产妇的会阴侧切率为30.5%,而对照组的会阴侧切率为62.0%,差异有统计学意义(P<0.05);两组产妇均未出现会阴Ⅲ级裂伤,其裂伤程度的差异有统计学意义(P<0.05);两组产妇第二产程时间的差异无统计学意义(P>0.05);观察组产妇产程疼痛级别明显低于对照组,差异有统计学意义(P<0.05)。结论会阴无保护接生可降低会阴侧切率,减少会阴切口感染,提高分娩舒适感,促进自然分娩率,值得临床推广应用。

Objective To study the effect of unprotected perineum delivery technique in spontaneous vagi- nal delivery of low risk pregnant women. Methods From January to June 2012,400 pimiparae with sponta- neous vaginal delivery in the hospital were enrolled in the study using convenience sampling. All the pimip- arae were randomized into observational group（n=200） and control group（n= 200） . Unprotected perine- um delivery technique and routine delivery technique were adopted in the pimparae in the observational and control groups retrospectively. The maternal perineum incision situation, perineal laceration, second process time and labor pain in the situation in the two groups were compared. Results The episitomy rates of the observational group and the control group were 30.5 % and 62 % respectively. The difference was statisti- cally significant（P〈0.05）. No perineal Ⅲ degree laceration occurred in both groups and the difference of laceration degrees was statistically significant（P〈0.05）. No significant difference was detected on the time of the second stage of labor between the two groups（P〉0.05）. The labor pain level of the observational group was significantly lower than that of the control group（P〈0.05）. Conclusion Unprotected perineum delivery technique can significantly decrease the rate of epiostomy, reduce the perineal incision infection, improve the comfort of deliver and promote the natural deliver,which deserves further clinical application.
