酚妥拉明联合多巴胺治疗老年典型左侧心力衰竭35例临床观察

Clinical Observation of Phentolamine Combined with Dopamine on Treatment of 35 Elderly Patients with Typical Left-heart Failure

目的探讨酚妥拉明联合多巴胺治疗老年典型左侧心力衰竭患者的疗效和安全性。方法收集2010年2月至2011年10月抚州市第一人民医院收治的老年典型左侧心力衰竭患者70例,按随机数字表法分为观察组和对照组各35例。对照组给予酚妥拉明治疗,观察组在此基础上加用多巴胺治疗。结果观察组总有效率为88.57%(31/35),对照组总有效率为68.57%(24/35),两组比较差异有统计学意义(P<0.05);两组给药2 h时心功能、血流动力学指标均较给药前显著改善,且观察组优于对照组(P<0.05);未发现不良反应。结论酚妥拉明联合多巴胺治疗老年典型左侧心力衰竭,能快速控制心力衰竭,改善血流动力学和心功能,显著提高疗效,安全系数大,尤其适用于老年患者。

Objective To investigate the efficacy and safety of phentolamine combined with dopamine on treatment of elderly patients with typical left heart failure. Methods 70 cases of elderly patients with typ- ical left heart failure collected from February 2010 to October 2011 in First People＇s Hospital of Fuzhou City, were divided according to the random number table into observation group and control group with 35 cases in each group. The control group received conventional treatment and were given phentolamine, the observation group used dopamine on the basis of the control group＇s treatment. Results The total effective rate of the ob- servation group was 88.57% （31/35 ）, significantly higher than that of the control group＇s 68.57% （24/ 35 ） ,the difference was statistically significant（ P 〈 0.05 ） ;the cardiac function indices and hemodynamics indices were significantly improved in both group at 2h of drug administration, and the observation group was significantly better than the control group （P 〈 0.05 ） ;no adverse reactions occurred in two groups. Conclu- sion The phentolamine combined with dopamine treatment of elderly patients with typical left heart failure can control heart failure rapidly, improve the hemodynamics and cardiac function, and significantly improve the efficacy,with high safety,which is especially suitable for the elderly patients.
