摘要
观察气流冲击法配合吸痰术对于行机械通气(MV)患者吸痰的效果及呼吸机相关性肺炎(VAP)的发生率。将59例行人工气道机械通气的患者随机分成两组,对照组采用常规的开放式口鼻咽部吸痰,实验组在常规吸痰的基础上采用简易呼吸器气流冲击法。比较两组患者的痰鸣音、日吸痰次数,连续两次吸痰时间间隔、机械通气时间、VAP发生率、住ICU时间。两组患者MV时间,差异有统计学意义(P=0.00)。两组患者痰鸣音改善情况及连续两次吸痰时间差异有统计学意义(P<0.05)。气流冲击法配合吸痰术可以有效清理患者气道分泌物,缩短患者的MV时间,减少肺部并发症。
To explore the effect of application of the impact of air on the prevention of ventilator-associated pneumonia (VAP) in mechanical ventilation patients (MV) and the result of suction by it. 59 patients on ventilator mechanically were randomly divided into 2 groups: one group received air impact and the other did not. To compare the two groups's effect of suctioning was evaluated by auscultation of the moist rales, the times of sputum suction in 24 hours and two consecutive sputum suction' time besides the time of treated with mechanical ventilation, ventilator associated pneumonia's incidence rate and days of stay in ICU were also observed and compared. There was significant difference in time of treated with MV (P=0.00), days of sputum suction and moist rales were also different between two groups (P〈0.05). Using air impact method to clear aspiration of subglottic secretions not only can shorten the MV average times but also can clean airway effi- ciently and extend the time of two consecutive sputum suction
