老年人高尿酸血症发病率及与心血管疾病危险因素相关性分析 被引量:10

摘要 目的探讨老年人高尿酸血症发生情况以及与心血管疾病之间的相关性。方法选择2009年7月至2011年12月在本社区卫生服务中心体检的老年人2894名,收集各项临床资料,根据其尿酸(uA)水平分为正常尿酸组和高血尿酸组,比较两组的临床资料,分析尿酸水平与心血管危险因素之间的相关性。结果老年人群高尿酸血症发生率为20.80%(602/2894)。高血尿酸组体重指数(BMI)、收缩压(SBP)、舒张压(DBP)、低密度脂蛋白胆同醇(LDL—C)、空腹静脉血糖(FBG)水平均显著高于血尿酸正常组,分别为(23.9±5.1)kg/m2比(19.2±3.2)kg/m2(P=0.020)、(146.2±12.8)mmHg比(134.8±8.6)mmHg(P=0.029)、(89.7±7.2)mmHg比(74.8±6.5)mmHg(P=0.016)、(3.18±0.69)mmol/L比(2.13±0.58)mmol/L(P=0.035)、(6.79±1.28)mmol/L比(5.05±1.12)mmol/L(P=0.039);而高密度脂蛋白胆同醇(HDL—C)水平显著低于血尿酸正常组[(1.19±0.28)mmol/L比(1.58±0.35)mmol/L(P=0.041)];两组血甘油三酯(TC)、血胆固醇(TG)水平差异无统计学意义(P〈0.05)。高血尿酸组高血压、高甘油三酯血症、高总胆同醇血症、糖尿病、超重及肥胖发生比例均显著高于血尿酸正常组,分别为10.5%比2.8%(P=-0.002)、5.1%比2.9%(P=O.026)、8.6%比5.0%(P=0.031)、4.7%比1.8%(P=0.013)、46.2%比24.8%(P=0,023oPearson相关分析显示,血尿酸浓度与BMI、SBP、DBP、TC、LDL—C呈显著正相关(P〈0.05),与HDL—C呈负相关(P〈0.05)。结论高血尿酸与高血压、高脂血症、糖尿病的发生存在密切的关系,从而增加了心血管疾病的危险性。 Objective To investigate the incidence of hyperuricemia in elderly people and its relationship with cardiovascular risk factors. Methods 2894 healthy elderly people from July 2009 to December 2011 were enrolled, all clinical date were received and all the people were divided into high blood uric acid group and normal blood uric acid group, the clinical date were compared in two groups and investigated the relationship between blood uric acid and cardiovascular risk factors. Results The incidence of hyperurieemia in elderly people was 20.80%(602/2894), the levels of BMI, SBP, DBP, LDL-C and FBG were significantly higher in high blood uric acid group than in normal blood uric acid group, that were (23.9±5.1)kg/m2 vs (19.2±3.2)kg/m2 (P=0.020), (146.2±12.8)mm Hg vs (134.8±8.6)mm Hg(P=0.029), (89.7±7.2)mm Hg vs (74.8±6.5)mm Hg(P=0.016), ( 3.18±0.69 )mmol/L vs ( 2.13±0.58 ) mmol/L(P=-0.035 ), ( 6.79± 1.28 )mmol/L vs ( 5.05± 1.12 )mmol/L(P=0.039 ), respectively. But the level of HDL-C was significantly lower in high blood uric acid group than in normal blood uric acid group, that was (1.19±0.28)mmol/L vs (1.58±0.35)mmol/L(P=O.041 ). And the levels of TC, TG were no difference between two groups(P〉0.05). The incidence of hypertension, hypertriglyceridemia, high total cholesterol, diabetes mellitus, overweight and obesity were significantly higher in high blood uric acid group than in norreal blood uric acid group, that were 10.5% vs 2.8%(P=0.002), 5.1% vs 2.9%(P=0.026), 8.6% vs 5.0%(P= 0.031 ), 4.7% vs 1.8%(P=0.013 ), 46.2% vs 24.8%(P=0.023), respectively. Pearson correlation analysis showed that the level of blood uric acid has positive relationship with BMI, SBP, DBP, TC and LDL-C (P〈0.05) and negative relationship with HDL-C (P〈0.05). Conclusion High blood uric acid has a significant relationship with the oeeurrenee of hypertension, hyperlipidemia and diabetes mellitus, so it increases the dangerous of cardiovascnlar disease.
