上皮钙粘素和基质金属蛋白-2在胃癌组织的表达及意义

The expression of E-cadherin and MMP2 in gastric carcinoma and its significance

目的探讨上皮钙粘素和基质金属蛋白酶-2(MMP2)在胃癌组织浸润和转移中的表达和意义。方法采用SP免疫组化法检测胃癌组织及胃良性病变中E-cadherin和MMP2表达情况。计数资料采用χ2检验,P<0.05为差异有统计学意义。结果 E-cadherin在胃癌中的表达低于胃部良性病变,在淋巴结转移组低于无淋巴结转移组(P<0.05),在浸润组的表达低于无浸润组的表达(P<0.05),在胃癌低分化组的表达低于高中分化组(P<0.05)。MMP2在胃癌中的表达高于胃部良性病变,在淋巴结转移组高于无淋巴结转移组(P<0.05),在浸润组的表达高于无浸润组的表达(P<0.05),在胃癌低分化组高于高中分化组(P<0.05)。同时,两者在胃癌中的表达存在相关性(P<0.05)。结论胃癌中存在E-cadherin下调和MMP2上调且两者具有相关性,两者的联合测定有助于判断胃癌患者的病期及预后。

Objective To discuss the expression and significance of E - eadherin and matrix metalloproteinase - 2 （MMP2） in the infiltration and metastasis of gastric carcinoma. Methods Gastric carcinoma lesions and benign lesions were detected for the expression of E - eadherin and MMP2 by immunohistochemical methods. Count data were processed with X2 test, the result of P 〈 0.05 was considered statistically significant. Results The expression of E - cadherin was lower in gastric carcinoma lesions than in benign lesions, lower in the group with lymph node metastasis than in the group without lymph node metastasis（P 〈0. 05）, lower in the group with infiltration than in the group without infiltration（P 〈0. 05 ）, and lower in poorly differentiated gastric cancer group than in well differentiated gastric cancer group （ P 〈 0. 05 ）. The expression of MMP2 was higher in gastric carcinoma lesions than in benign lesions, higher in the group with lymph node metastasis than in the group without lymph node metastasis （ P 〈 0. 05 ）, higher in the group with infiltration than in the group without infiltration （ P 〈 0. 05 ）, and higher in poor differentiated gastric cancer group than in well differentiated gastric cancer group（P 〈0.05）. At the same time, the expression of both E - cadherin and MMP2 in gastric carcinoma was correlated （ P 〈 0.05 ）. Conclusions Down - regulation of E - cad_herin and up - regulation of MMP2 occur in gastric carcinoma and are correlated with each other. The combined detection of E - cadherin and MMP2 is helpful in determining the stage and prognosis of gastric carcinoma.
