摘要
目的探讨中耳功能对耳声发射结果的影响及高频鼓室声导抗在婴儿听力筛查中的意义。方法对288例(526耳)无听力损失高危因素婴儿行DPOAE及1kHz鼓室声导抗测试,分析耳声发射通过与未通过者的中耳功能差异。结果 526耳中,DPOAE通过374耳,1kHz鼓室声导抗异常率为7.49%(28/374),其中,全频通过245耳,部分频率通过129耳;DPOAE未通过152耳,1kHz鼓室声导抗异常率为57.89%(88/152);DPOAE部分频率通过者的高频鼓室声导抗异常率(14.73%,19/129)高于全频率通过者(3.67%,9/245)(P<0.001),DPOAE未通过者的高频鼓室声导抗异常率(57.89%)高于部分频率通过者(14.73%)(P<0.001)。结论 1kHz鼓室声导抗检测应作为婴儿听力筛查的重要辅助手段,其对听力损失性质的诊断及追踪具有重要意义。
Objective To investigate the effects of middle ear function on the otoacoustic emissions and the significance of application of high-frequency tympanometry in hearing screening. Methods High-frequency 1 kHz tympanometry and otoacoustic emissions were obtained from 288 infants(526 ears) without hearing loss risk factors.The differences of middle ear function in infants with otoacoustic emissions 'pass' and 'refer' were studied. Results Anong 526 ears,374 ears passed DPOAE in which 1 kHz tympanometry showed that 28 ears(7.49%,28/374) abnormal.While,152 ears failed DPOAE in which 88 ears(57.89%,88/152) were abnormal in 1 kHz tympanometry.Partd frequenits in 129 ears passed DPOAE,and 19 ears out of these 129 ears(19/129,14.73%) showed abnormal by 1 kHz tympanometry.Full frequevcies in 245 ears passed DPOAE,and only 1 out of 245 ears(9/245,3.67%) showed abnormal by 1 kHz tympanometry.The abnormal rates in 1 kHz tympanometry between these 2 groups were signifiant different(P<0.001).Also the abnormal rate by 1 kHz tympanometry in DPOAE failed group was 57.89%,which was signifiantly higher than that of in partal frequcies DPOAE passed group(14.73%)(P<0.001). Conclusion High frequency 1 kHz tympanometry could be used as an assistant mean for hearing screening,it was important for evaluating the nature of hearing loss and the troctment of hearing loss.
