急性重型胰腺炎患者血液流变性的变化及其相互关系

Hemorrheological changes in patients with severe acute pancreatitis

目的 :探讨血液流变性变化在急性重型胰腺炎 (SAP)微循环障碍中的作用 ,并分析 SAP血液流变性各参数间的相互关系。方法 :采用 FASCO 94全自动粘度快测仪测定血液流变性各参数。结果 :入院 2 4小时内 SAP患者血液流变性各参数显著增高 ;而急性轻型胰腺炎 (MAP)患者与健康对照 (N)组比较无明显差异。SAP患者高切变率下全血粘度与血细胞比容和红细胞变形指数呈显著正相关 ;中切变率下全血粘度与血细胞比容、红细胞聚集指数和红细胞变形指数呈明显正相关 ;低切变率下全血粘度与血细胞比容、红细胞聚集指数呈显著正相关 ;血细胞比容与红细胞聚集指数和红细胞变形指数呈明显正相关。结论 :MAP患者血液流变性无明显异常 ,而 SAP患者血液流变性则有显著异常 ,各参数间相互影响 ,全血粘度和血细胞比容明显升高 ,红细胞聚集性增强及红细胞变形能力下降是导致

Objective:To explore the role of hem orrheological changes in pancreatic m icrocirculatory distur- bance in patients with severe acute pancreatitis(SAP) .Methods:The param eters of hemorrheology were mea- sured by FASCO 94full automatic viscosity fast determination instrument.Results:All hem orrheological pa- rameters in SAP group were higher than those in other groups.There was no significantdifference in these pa- rameters between mild acute pancreatitis(MAP) and healthy control groups.Whole blood viscosity in high shear in SAP group was significantly positively correlated with hemotocrit and erythrocyte deform ability in- dex.Whole blood viscosity in middle shear had distinctly positive correlations with hemotocrit,aggregation of erythrocytes index,and erythrocyte deform ability index.Whole blood viscosity in low shear significantly posi- tively correlated with hemotocrit and aggregation of erythrocytes index,and hemotocrit was significantly posi- tively correlated with aggregation of erythrocytes index and erythrocyte deform ability index.Conclusions:The results from hemorrheological parameters showed significant changes in SAP patients and no obvious changes in MAP patient.The increase of blood viscosity and hem otocrit,the enhancementof the tendency of erythrocyte aggregation and the decrease of erythrocyte deform ability are causative factors of pancreatic m icrocirculatory disturbances in SAP.
