摘要
目的研究血清载脂蛋白AV(apolipoprotein AV,apoAV)水平及其与甘油三酯的关系在脑梗死患者和正常人群之间的差异,并观察脑梗死急性期血清apoAV水平的动态变化。探讨apoAV基因-1131 T>C多态性与血清apoAV水平、甘油三酯水平及脑梗死之间的关系。方法采用病例对照研究,在湖南汉族人群中筛选123例急性期脑梗死患者及113例年龄和性别匹配的健康体检人群为研究对象。应用双抗体夹心ELISA方法测定研究对象血清apoAV水平。采用聚合酶链反应一限制性片段长度多态性(PCR-RFLP)法检测apoAV-1131 T>C基因型。结果研究总人群中血清apoAV水平与甘油三酯水平呈负相关(rs=-0.132,P<0.05)。病例组中血清apoAV水平与脑梗死NIHSS评分间有呈正相关的趋势,但无统计学意义(rs=0.161,P>0.05)。ApoAV-1131 T>C各基因型间血清apoAV水平无显著性差异(均P>0.05)。多因素logistic回归分析显示apoAV-1131 C等位基因是脑梗死的独立危险因素(OR=1.897,95%CI 1.029~3.499,P<0.05)。结论在湖南汉族非脑梗死人群中,血清apoAV具有降低甘油三酯水平的作用,其水平在急性脑梗死患者与正常人群之间无明显差异。ApoAV-1131 C等位基因是脑梗死的独立危险因素,其与甘油三酯水平升高有关,但与apoAV水平无关。
Objective To explore the difference in the level of serum apolipoprotein AV (apoAV) between cerebral infarction patients and normal population and the relationship between the levels of serum apoAV and triglyceride. To inves- tigate the gene frequencies of apoAV-1131T 〉 C gene polymorphism, and the association between apoAV-1131 T 〉 C gene polymorphisrri with the levels of serum apoAV,triglyceride or cerebral infarction. Methods A case-control study on 123 patients with cerebral infarction and 113 healthy controls was carried out. The level of serum apoAV was determined by double antibody sandwich ELISA. The genotype and allele frequencies of apoAV-1131 T 〉 C gene polymorphism were as- sayed by polymerase chain reaction-restricted fragments length polymorphism (PCR-RFLP). Results Serum apoAV con- centration was negatively correlated with triglyceride( r = - 0. 132, P 〈 0.05 ). The trend of positive correlation was found between the level of serum apoAV and NIHSS seore,but no statistically significance( rs =0. 161 ,P 〉 0.05). There was no significant difference of serum apoAV level among apoAV-1131 T 〉 C genotypes. ApoAV-1131 C allele was independently associated with an increased risk of cerebral infarction( OR = 1. 897,95% CI 1. 029 - 3. 499,P 〈 0.05 ). Conclusion Ser- um apoAV reduced the level of triglyceride in the normal population, but did not in the acute stage of cerebral infarction. ApoAV-1131 C allele is an independent risk factor for cerebral infarction, but theit increased serum triglyceride level was not through changing the level of serum apoAV.
