摘要
目的调查20株泛耐药鲍曼不动杆菌(pandrug-resistant Acinetobacter baumannii,PDR-ABA)的菌株亲缘性。方法收集2010年3月到2010年5月临床标本中分离的PDR-ABA菌20株,采用聚合酶链反应(PCR)的方法检测3种管家基因和37种β-内酰胺类获得性耐药基因、15种氨基糖苷类获得性耐药基因、5种喹诺酮类获得性耐药基因和8种可移动遗传元件等水平转移基因检测,并对检测结果作了样本聚类分析。结果本组PDR-ABA菌carO、gyrA和parC3种管家基因均存在突变,且突变形式一致;blaTEM、blaADC-like、blaOXA-23、aac(6')-Ⅰb、ant(2")-Ⅰ、ant(3")-Ⅰ、aphA1、adeB、tnpU、ISCR1、IS26和ISaba1基因全部阳性,blaCARB-2、blaDHA-1和IS903基因阳性率为5%。样本聚类分析显示本组PDR-ABA菌为克隆传播。结论本组PDR-ABA菌为医院内克隆传播,应加强控制。
Objective To investigate the phylogeny of 20 strains of pandrug-resistant Acinetobacter baumannii (PDR-ABA). Methods From March 2010 to May 2010, 20 strains of PDR-ABA were collected from clinical specimen. Then, 3 kinds of housekeeping genes, 57 kinds of acquired resistance-related genes to beta-lactams, aminoglycosides, quinolones and 8 kinds of genetic markers of mobile genetic element were analyzed by PCR and the phylogenetic analysis was performed. Results All 20 strains of PDR-ABA were existed mutations for 3 kinds of housekeeping genes and positive for blarEM, b[anoc.like, blaoxn.23, aac(6')- I b, ant(2 ")- I, ant(3 ")- I, aphA1, adeB, tnpU, ISCR1, IS26, 1Sabal, and the positive rate of genes of blaCARB-2, blaDKA.1 and IS903 were 5.0%. Sample cluster analysis showed that this group of PDR-ABA were clonal spread. Conclusions This group of PDR-ABA were clonal spread and the outbreak should be controlled.
