摘要
目的开展广东省五华县乙肝病毒表面抗(HBsAg)原阳性人群的转归及其家庭成员乙肝病毒(HBV)感染情况调查,为乙型肝炎防控提供科学依据。方法 2010-01对在2006年血清学调查中发现的HBsAg阳性人群家庭成员进行初筛,筛查出HBsAg阳性者。对2006年发现的HBsAg阳性人群及其HBsAg阳性家庭成员开展体征检查,B超检查肝脏、脾脏和胆囊,进行肝功能及HBV相关指标检测分析。结果在原59名HBsAg阳性人群中,有5名HBsAg阳性人员已转为阴性,HBsAg阴转率为8.47%,HBV携带者47例(79.66%),慢性乙肝7例(11.86%);肝功能检查显示丙氨酸氨基转移酶(ALT)升高14例(23.73%),天门冬氨酸氨基转移酶(AST)升高15例(25.42%),B超检查显示脂肪肝6例(10.17%)。筛查检测59名原HBsAg阳性人群的127名家庭成员,检出HBsAg阳性26例,家庭成员HBsAg阳性率20.47%,乙肝病毒感染聚集家庭病毒传播关系以母亲阳性孩子阳性为主,有12人次,占46.16%。结论五华县农村HBsAg阳性人群家庭成员乙肝病毒感染率略高,家庭中传播途径以母子(女)为主,提示今后应重点关注HBsAg阳性人群家庭的乙肝免疫策略。
Objective To investigate the prognosis of hepatitis B virus surface antigen (HBsAg) positive population and hepatitis B virus (HBV) infection status among their family members in Wuhua county, thus to provide the scientific basis for control and prevention of hepatitis B. Methods In Jan. 2010, serological survey was carried out among family members of HBsAg positive population ascertained in 2006 HBsAg. Indexes pertaining to liver, spleen, gallbladder B ultrasound and liver function were examined among HBsAg positive population and their family members. Results In the original 59 HBsAg positive cases, 5 cases turned to negative, with the HBsAg negative conversion rate of 8.47%. The total number HBV carriers was 47 (79.66%) ; and chronic hepatitis B, 7 cases (11.86%). The liver function examination showed that: ALTrose in 14 cases (23.73%) ; AST rose in 15 cases (25.42%). B ultrasound examination showed that 6 cases had fatty liver ( 10. 17% ). The 127 family members of 59 original HBsAg positive cases were detected, among which, 26 cases were HBsAg positive (20. 47% ). The transmission route was mainly on positive mother to positive child, and the total number was 12 (46. 16% ). Conclusion The HBV infection rate amongHBsAg positive population is somewhat high that expeeted; the route of transmission is mainly mothers totheir offspring, reminding a close watch should be paidon the family members.
