摘要
目的探讨超声心动图对感染性心内膜炎的诊断价值,为感染性心内膜炎的诊断提供参考。方法共纳入85例诊断为感染性心内膜炎患者作为病例组,同时随机抽取90例无心脏疾病史的人群作为对照组,所有患者均行超声心动图检查,评价其对感染性心内膜炎的诊断价值。结果超声心动图敏感度为56.5%,特异度为71.1%,阳性似然比为0.65,阴性似然比0.37;超声心动图检查结果中共有74例受试者检查有赘生物,最大24mm×19mm,最小3mm×3mm;其中附着于主动脉瓣45例、二尖瓣27例、三尖瓣4例、肺动脉瓣4例、主动脉瓣和二尖瓣3例,人工瓣2例。结论超声心动图对感染性心内膜炎有比较高的诊断特异度,临床上可以早期对患者进行检查,为早期的治疗提供参考依据,值得临床推广应用。
OBJECTIVE To explore the clinical value of echocardiography in diagnosis of infective endocarditis so as to provide basis for the clinical diagnosis of the infective endocarditis.METHODS Totally 85 patients who were diagnosed as echocardiography patients were enrolled in the treatment group,and 90 participants without heart disease history were randomly selected as the control group.All the patients underwent the echocardiography,and its clinical value in the diagnosis of the infective endocartis was evaluated.RESULTS The sensitivity of the echocardiography was 56.5%,the specificity was 71.1%,positive likelihood ratio was 0.65,negative likelihood ratio was 0.37.The echocardiography results found that there were 74 subjects who were checked with the vegetations,the biggest 24 mm × 19 mm,the smallest 3 mm × 3 mm.There were 45 cases with the vegetations attached on the aortic valve,27 cases on the mitral valve,and 4 cases on tricuspid 4 cases,4 cases on pulmonary valve,3 cases on aortic valve,3 cases on mitral valve,and 2 cases on artificial disc.CONCLUSION Echocardiography has a highest specificity in the diagnosis of the infective endocarditis,which can contribute the early clinical diagnosis of the patients and provide the basis for the early clinical treatment.It is worthy of the clinical promotion.
