普外科腹部切口感染相关因素及预防对策

Related factors and prevention countermeasures forgeneral surgery abdominal incision infections

目的探讨普外科腹部切口感染相关因素及预防措施,以预防控制手术切口感染率。方法回顾性分析2008年1月-2011年12月2450例患者的临床资料,根据切口感染诊断标准进行判断,比较合并切口感染及无感染患者在性别、年龄、体质量指数、接台手术、切口长度、手术时间、应用抗菌药物等差异。结果 2450例患者发生切口感染80例,感染率为3.27%,行logistic回归分析影响切口感染因素包括:年龄≥60岁、体质量指数≥24.0、接台手术手术时间≥2h、术后应用抗菌药物,4年发生切口感染率分别为5.31%、5.68%、5.37%、6.16%、5.45%;OR值分别为3.87、3.56、2.97、3.01、2.89。结论普外科腹部切口感染是多因素作用的结果,应针对上述因素采用针对性预防对策。

OBJECTIVE To explore the related factors and prevention countermeasures for the abdominal incision infections due to the general surgery.METHODS The clinical data of 2450 patients were retrospectively analyzed from Jan 2008 to Dec 2011.The incision infections were judged according to the diagnosis criteria,the differences in the gender,age,body mass index,connecting surgery,incision length,operation duration,and application of antibiotics between the patients with incision infections and the patients without infections were compared.RESULTS The incision infections occurred in 80 of 2450 patients with the infection rate of 3.27%,the logistic regression analysis showed that the age of more than 60 years,body mass index more than 24.0,connecting surgery,operation duration more than 2 hours,and use of antibiotics after the surgery were the related factors for the incision infections.The incidence rates of the incision infections in the four years were 5.31%,5.68%,5.37%,6.16% and 5.45%,respectively,and the OR value were 3.87,3.56,2.97,3.01 and 2.89,respectively.CONCLUSION There are a variety of factors acting together to result in the general surgery abdominal incision infections,it is necessary to take the targeted prevention countermeasures aiming at the factors mentioned above.
